بررسی کلینیکی میزان موفقیت بازسازی داخل دهانی ترمیمهای چینی باند شونده به فلز

چکیده

مقدمه
رستوریشنهای متال سرامیک، علیرغم پیشرفت روزافزون در علم مواد، از نظر استحکام و باندینگ هنوز در 5% موارد بعد از 10 سال دچار شکستگی می‌شوند. هدف تحقیق ما انتخاب روش ترمیم چینی مناسبی است که باندینگ بهتر، استحکام و زیبایی بیشتر، مراحل ساده تر و عمر طولانی تر داشته باشد.
مواد و روش‌ها
در مطالعه حاضر که به روش In vivo انجام گرفته است، بیست دندان پره مولر فک بالا انتخاب شد. بعد از ساخت روکش به طریقه معمول اقدام به برداشتن قسمتی از چینی نیمه باکالی دندان نمودیم. این کار به صورتی انجام شد که در نیمی از نمونه‌ها، تنها چینی اکسپوز شده و در نیم دیگر فلز و چینی هر دو اکسپوز شدند. روکشها در دهان بیماران بوسیله سمان موقت Temp-Bond چسبانده شدند.
سپس ناحیه شکسته شده با استفاده از سیستم ترمیم چینی عرضه شده توسط کارخانه ویوادنت بازسازی شدند. بیماران در دوره‌های 2 هفته، 1 ماه، 3 ماه، 6 ماه و 9 ماه از نظر موفقیت و دوام ترمیم کنترل شدند و پس از جمع آوری نتایج، داده‌ها مورد ارزیابی و نتیجه گیری قرار گرفتند.
یافته‌ها
درصد موفقیت 9 ماهه در گروه اکسپوز چینی 80% و در گروه اکسپوز چینی- فلز100% و میزان موفقیت کلی مطالعه 90% محاسبه شد.
نتیجه‌گیری
ترمیم داخل دهانی روکشهای PFM با استفاده از سیستم ویوادنت میتواند جایگزین ساخت مجدد روکش و یا چینی گذاری مجدد شود و موفقیت آن در طولانی مدت بخصوص در گروه اکسپوز چینی- فلز قابل قبول است.

Introduction: In spite of progress in dental material science, considering strength and bonding, PFM restorations still have 5% failure after 10 years. The main aim of this investigation was to choose a system with better bonding, more strength and esthetics, simpler technique and more durability.
Materials & Methods : In this invivo investigation, twenty upper premolars were selected. The crowns were made in the usual manner and then a layer of porcelain was removed using stone from buccal cusp. In half of the samples both metal and porcelain were exposed and in the other half metal was not exposed. The crowns were cemented with Temp-Bond and then the fractured area was repaired intraorally with Vivadent system. The patients were recalled after 2 weeks, 1, 3,6 and 9 months to check durability and success rate of the restoration. Finally the data were analysed statistically.
Results : We calculated the success rate of restoration to be 80% in porcelain-exposed group, 100% in porcelain-metal-exposed group and 90% totally.
Conclusion : Intraoral porcelain repair, with the use of Vivadent system can be used instead of removing and remaking fractured crowns with satisfactory durability in long terms.
