مقایسهی تغییرات اکلوژن در بیماران ارتودنسی پس از استفاده از ریتینرهای Essix و هالی توسط T-Scan III
مقدمه : ثبات اکلوژن پس از درمانهای ارتودنسی از اهمیت زیادی برخوردار بوده و این ثبات پس از فاز فعال درمان توسط دستگاههای نگهدارنده به دست میآید. ریتینر هالی کاربرد زیادی برای ریتنشن در درمانهای ارتودنسی دارد؛ ولی امروزه تمایل به استفاده از دستگاههای ریتینر شفاف افزایش یافته است. تحقیق حاضر با هدف مقایسهی ستلینگ اکلوژن در بیماران ارتودنسی متعاقب استفاده از ریتینرهای Essix و هالی با استفاده از روش دیجیتالی T-Scan III انجام شد.مواد و روش ها: در این تحقیق مقطعی-تحلیلی، 46 بیمار ارتودنسی در انتهای فاز فعال درمان انتخاب و به دو گروه هالی (24 نفر) و Essix (22 نفر) تقسیم شدند. اکلوژن بیماران در ابتدای دوره، 6 ماه و 12 ماه بعد از استفاده از ریتینرها با استفاده از دستگاه ثبت اکلوزال دیجیتالی (T-Scan III) ارزیابی گردید. تغییرات درصد تماسهای اکلوزالی در زمانهای مختلف، در داخل گروهها با آزمون t زوجی و در بین گروهها با آزمون t مستقل مورد قضاوت آماری قرار گرفت.یافتهها: در گروه ریتینر هالی، درصد تماسهای اکلوزالی در دندانهای خلفی به میزان میانگین 05/13±708/7 درصد افزایش و در دندانهای قدامی با میانگین 791/11±416/7 درصد کاهش یافت. این مقادیر در گروه ریتینر Essix به ترتیب در دندان های خلفی برابر 884/14±19/10 درصد کاهشی و در دندان های قدامی 591/18±381/10 درصد افزایشی برآورد شد.نتیجهگیری: در مجموع، استفاده از هر دو ریتینر هالی و Essix منجر به تغییرات تماسهای اکلوزالی و حرکت عمودی نسبی دندانهای قدامی و خلفی در بیماران در دورههای زمانی 6 ماه و 1 سال پس از ریتنشن نسبت به ابتدای دوره شد ولی میزان تغییرات دردندان های خلفی در ریتینر هالی بیشتر از ریتینر Essix بود. با توجه به افزایش تماس های دندانی خلفی و کاهش تماس دندان های قدامی با استفاده از ریتینر هالی شاید بتوان این ریتینر را بیشتر در موارد دیپ بایت استفاده کرد. همچنین با توجه به کاهش تماس های خلفی در 6 ماهه اول در پلاک Essix پیشنهاد می شود مدت استفاده تمام وقت این ریتینر کوتاه تر شود.

بررسی آگاهی و عملکرد دندانپزشکان عمومی یزد در مورد مالتیپل اسکلروزیس در سال 1399
مقدمه: مالتیپل اسکلروزیس به عنوان یک بیماری نورودژنراتیو، تظاهرات دهانی-صورتی متعددی دارد. این عوارض می تواند توانایی بیمار برای حفظ بهداشت دهان و همکاری در مطب دندانپزشکی را با مشکل مواجه کند. مطالعه حاضر، با هدف بررسی آگاهی و عملکرد دندانپزشکان عمومی در مورد مالتیپل اسکلروزیس در یزد انجام شد.مواد و روشها: در این مطالعه ی توصیفی-مقطعی، 105دندانپزشک عمومی شاغل در شهر یزد بطور تصادفی انتخاب شدند و میزان آگاهی و عملکرد درخصوص درمان های دندانپزشکی بیماران دارای مالتیپل اسکلروزیس با استفاده از یک پرسشنامه محقق ساخته حاوی مشخصات دموگرافیک، 13سوال آگاهی و 10 سوال عملکرد، مورد مطالعه قرار گرفت. داده ها به کمک آزمون های آماری t و ضریب همبستگی پیرسون تجزیه و تحلیل شدند.یافتهها: از 105 دندانپزشک شرکت کننده در مطالعه، 7/46% مرد و 3/53% زن بودند. میانگین سنی آنها 47/4±09/30 سال و میانگین سابقه کارشان 59/3±84/4 سال بود. میانگین نمره آگاهی دندانپزشکان 54/2±39/14 (بیشترین نمره ممکن 26) و میانگین نمره عمکرد آنها 16/2±45/12 ( بیشترین نمره ممکن 20) بود. ارتباط بین نمرات آگاهی و عملکرد مستقیم و معنادار بود (232/0=r، 022/0=P). تفاوت آماری معناداری بین میانگین نمره آگاهی و عملکرد دندانپزشکان بر حسب جنس، گروه سنی و سابقه کارمشاهده نشد (05/0<P).نتیجهگیری: بر اساس نتایج مطالعه حاضر، میزان آگاهی و عملکرد دندانپزشکان در مورد بیماران مبتلا به مالتیپل اسکلروزیس در حد کم تا متوسط بود.بنابراین همکاری بین رشته ای در برگزاری دوره های باز آموزی جهت بالاتر بردن سطح آگاهی و متعاقب آن، عملکرد دندانپزشکان عمومی ضروری است.

بررسی کیفیت زندگی وابسته به سلامت دهان در بیماران مبتلا به مالتیپل اسکلروزیس
مقدمه: مالتیپل اسکلروزیس (MS) یک بیماری التهابی تخریب کننده ی نورون ها است که علاوه بر عوارض جسمی و روانی، بر سلامت دهان اثرات مخرب دارد. با وجود بررسی های بسیار در زمینه ی سلامت دهان در بیماران مبتلا به MS، تا به حال مطالعه ای به بررسی کیفیت زندگی وابسته به سلامت دهان در این بیماران نپرداخته است. هدف از این مطالعه بررسی کیفیت زندگی وابسته به سلامت دهان در بیماران مبتلا به MS بود. مواد و روش ها: در مطالعه ی مورد-شاهدی حاضر، تعداد 103 بیمار مبتلا به MS و 103 فرد سالم حضور داشتند. افراد هر دو گروه پرسشنامه های استاندارد شده Oral Health Impact Profile (OHIP) و ارزشیابی شاخص های مرتبط با رفتار سلامت دهان را تکمیل کردند. همچنین شاخص پوسیدگی برای افراد هر دو گروه اندازه گیری و ثبت گردید. در نهایت داده ها در نرم افزار SPSS با استفاده از آزمون های Mann-Whitney و T-test و رگرسیون خطی بین متغیر ها آنالیز شدند.یافته ها: کیفیت زندگی وابسته به سلامت دهان در گروه بیماران مبتلا به MS از جنبه ی محدودیت عملکرد از نظر آماری بطور معنی داری پایین تر از گروه سالم بود (032/0=P). علاوه بر آن، نمره ی شاخص پوسیدگی در بیماران مبتلا به MS بطور معنی داری بالاتر بود (025/0=P). نتیجه گیری: به نظر می رسد کیفیت زندگی وابسته به سلامت دهان در بیماران مبتلا به MS در مقایسه با افراد سالم، علی رغم داشتن پوسیدگی بیشتر، تفاوت معنی داری ندارد. مطالعات کیفی به منظور بیان بهتر مشکلات توسط این بیماران در این زمینه نیاز است.

بررسی اثر آماده سازی های سطحی مختلف و دو سیستم ترمیم پرسلن بر روی استحکام باند برشی کامپوزیت به زیر کونیا
مقدمه: شکست پرسلن ونیر، یکی از مشکلات رستوریشن های زیرکونیا می باشد.ترمیم داخل دهانی با کامپوزیت میتواند جایگزین تعویض ترمیم باشد. باند با ثبات میان کامپوزیت و زیرکونیا جهت دوام ترمیم ضروری می باشد. هدف از مطالعه حاضر، بررسی اثر آماده سازی های سطحی مختلف و دو سیستم ترمیم پرسلن داخل دهانی بر روی استحکام باند برشی کامپوزیت به زیرکونیا بود.مواد و روش ها: در این مطالعه تجربی، 80 دیسک زیرکونیا (ضخامت 3 و قطر 6 میلیمتر) بطور تصادفی به 4 گروه تقسیم شدند: گروه کنترل، سندبلاست با سیستم Cojet، تابش با لیزر Er:YAG و تابش با لیزر Nd:YAG. در هر گروه نمونه ها با دو سیستم ترمیم پرسلن Bisco و Clearfil آماده شدند. کامپوزیت بر روی دیسک های زیرکونیا قرار گرفت و کیور شد. پس از ترموسیکلینگ، استحکام باند برشی نمونه ها توسط دستگاه تست یونیورسال بدست آمد. داده ها با آزمون های Two way ANOVAو Tukey HSD تحلیل گردیدند. ( 05/0=α)یافته ها: میانگین استحکام باند برشی در محدوده 26/1 ± 52/4 تا 56/3 ± 79/13مگاپاسکال بود. اثر آماده سازی سطحی براستحکام باند برشی معنی دار بود (001/ 0P<). ولی مقدار استحکام باند بر حسب سیستم ترمیم تفاوت معنی دار آماری نداشت. (56/0=P)نتیجه گیری: سندبلاست سطح زیرکونیا با سیستم Cojet استحکام باند به کامپوزیت را افزایش داد. آماده سازی با دو نوع لیزر، تاثیری در افزایش استحکام باند نداشت. دو سیستم ترمیم پرسلن Clearfil) و (Bisco تاثیر مشابهی بر استحکام باند داشتند.

بررسی تغییرات دیسپلاستیک کیست های پری اپیکال باقی مانده در آرشیو بخش آسیب شناسی دهان دانشکده دندانپزشکی دانشگاه علوم پزشکی اصفهان در یک دوره ۳۰ ساله (97-1368)
مقدمه: احتمال تغییرات دیسپلاستیک حاصل از کیست پری اپیکال باقیمانده بیشتر از سایرکیست های ادونتوژنیک ذکر شده است. باتوجه به اهمیت این موضوع، مطالعه حاضر با هدف تعیین میزان تغییرات دیسپلاستیک کیست های پریاپیکال باقی مانده انجام گرفت.مواد و روش ها: در مطالعه مقطعی توصیفی- تحلیلی حاضر، کلیه پرونده های افرادی که به صورت قطعی تشخیص کیست پری اپیکال باقیمانده برای آن ها ثبت شده بود، از آرشیو بخش آسیب شناسی دهان و فک و صورت دانشکده دندانپزشکی دانشگاه علوم پزشکی اصفهان دریک دوره سی ساله (97-1368) استخراج شد. سپس اسلایدهای میکروسکوپیک رنگ آمیزی شده با هماتوکسیلین-ائوزین حاصل از این پرونده ها که حاوی ضایعات پری اپیکال باقیمانده بودند، از نظر میزان تغییرات دیسپلاستیک توسط دو آسیب شناس دهان و فک و صورت به طور همزمان بوسیله میکروسکوپ نوری بررسی شدند. داده ها پس از گردآوری توسط نرم افزار SPSS 22 ، آمار توصیفی و آزمون های آماری کای اسکوئر، دقیق فیشر و t-student تجزیه و تحلیل شدند (α=0.05).یافته ها: اسلایدهای ۱۹۷ بیمار از آرشیو استخراج شدند که از آن میان، اسلایدهای ۱۹ بیمار به دلیل مخدوش بودن، از مطالعه خارج گردید و درنهایت اسلایدهای 178 بیمار آنالیز شد. تغییرات دیسپلاستیک مشاهده شده در 16 مورد خفیف و دریک مورد متوسط بود. توزیع فراوانی دیسپلازی براساس سن، جنس و محل آناتومیک معنی دارنبود.نتیجه گیری: در مطالعه حاضر، میزان شیوع دیسپلازی در کیست های پری اپیکال باقیمانده کم (حدود ده درصد) بود، اما با توجه به بوجود آمدن سرطان ناشی از تغییرات دیسپلاستیک، انجام تحقیقات بیشتر پیشنهاد می گردد.

اجرا و ارزشیابی نرم افزار آموزشی زخم های دهانی با قابلیت اجرا بر روی تلفن های هوشمند
مقدمه: هدف این مطالعه، ارزیابی میزان استفاده و رضایت دانشجویان از نرم افزار آموزشی Moodle mobile با قابلیت اجرا بر روی تلفن های هوشمند تحت سیستم عامل ios و Android برای واحد درسی بیماریهای دهان فک و صورت عملی بود.مواد و روش ها: نرم افزار آموزشی با امکانات نمایش اطلاعات مدرسین و راه های ارتباطی با آنها، نمایش داده های آموزشی مختلف و توانایی برگزاری آزمون طراحی شد. 46 دانشجو، شامل 20 دختر (43%) و 26 پسر (57%) که واحد درسی بیماری های دهان عملی 2 را در نیم سال اول تحصیلی 96-1395 انتخاب کرده بودند، در این مطالعه شرکت کردند. افراد به دو گروه دارای سیستم عامل android وios تقسیم شدند. در پایان، میزان استفاده و همچنین رضایت دانشجویان از این نرم افزار به تفکیک جنس با استفاده از چک لیستی ارزیابی شد.یافته ها: سیستم عامل تلفن همراه بیشتر شرکت کنندگان اندروید بود. کمترین تعداد مراجعه به تلفن همراه در شبانه روز، هر 120 دقیقه یکبار و بیشترین تعداد مراجعه هر 3 دقیقه یکبار بوده است. دختران تقریبا 5/1 برابر پسران، بیش از دو ساعت در شبانه روز از تلفن همراه استفاده می کردند. بیشترین استفاده کاربری (80 درصد) فقط برای شبکه های اجتماعی بوده است. بیشترین استفاده ی آموزشی مربوط به رشته ی تخصصی، نیم ساعت بود. میانگین استفاده از این نرم افزار در شبانه روز در دختران حدود 80 دقیقه و در پسران حدود 62 دقیقه بود. در شرایط حضور در بخش، بیشترین استفاده مربوط به نمایش داده های آموزشی بود. از 34 درصدی که به این نرم افزار نقد داشتند، 50 درصدشان از کافی نبودن اسلاید ناراضی بودند.نتیجه گیری: این نرم افزار، از پذیرش خوبی میان دانشجویان دندانپزشکی برخوردار بود و یک سیستم کمک آموزشی الکترونیک از آن در هر زمان و هر مکانی با ویژگی و امکانات مختلف در جهت تشخیص و بهبود یادگیری ضایعات دهانی در دسترس بود.

بررسی مورفولوژی مولرهای دوم ماگزیلا با ریشه های به هم چسبیده به وسیله توموگرافی کامپیوتری با اشعه مخروطی
مقدمه: مهمترین علت درمان ریشه ناموفق، آماده سازی و پرکردگی ضعیف کانال ها و همچنین کانال های درمان نشده می باشد. بنابراین شناخت آناتومی سیستم کانال ریشه از ارکان اصلی موفقیت درمان ریشه محسوب میشود. هدف این مطالعه بررسی مورفولوژی سیستم کانال ریشه دندان های مولر دوم ماگزیلا با ریشه های به هم چسبیده بود.مواد و روش ها: در این مطالعه آزمایشگاهی، 50 دندان مولر دوم ماگزیلا با ریشه های به هم چسبیده مانت شده و از آنها توموگرافی کامپیوتری با اشعه مخروطی (CBCT) تهیه شد. نمونه ها از نظر سیستم کانال ریشه مزیوباکال بر اساس تقسیم بندی Vertucci، وجود کانال مزیوباکال دوم و سوم، وجود ایسموس بین کانالها، آناتومی C شکل، وضعیت اتصال ریشه ها و حداقل ضخامت عاج در یک سوم میانی بررسی شدند. برای آنالیز داده ها از آزمون های کروسکال والیس، یوی من ویتنی و آزمون دقیق فیشر استفاده شد و سطح معنی داری 05/0 در نظرگرفته شد.یافته ها: شایعترین آناتومی، کانال تایپ 1 با فراوانی 42% (21 عدد) و سپس تایپ 4,6 با فراوانی هرکدام 16% (8 مورد) و تایپ 2 با فراوانی 14% (7 مورد) بود. کانال مزیوباکال دوم در 58% (29 مورد) و کانال مزیوباکال سوم در 4% (2 مورد) مشاهده شد. در 22% دندان ها (11 مورد) بین کانال ها ایسموس مشاهده شد. 6% دندان (3 مورد) آناتومی C شکل داشتند. شایعترین وضعیت اتصال ریشه ها مربوط به اتصال ریشه پالاتال و دیستوباکال با شیوع 42% (21 مورد) و سپس اتصال هر سه ریشه با فراوانی 18% (9 مورد) بود. حداقل ضخامت عاج در یک سوم میانی ریشه 64/0 میلیمتر تعیین شد.نتیجه گیری: مولر های دوم ماگزیلا با ریشه های به هم متصل، سیستم کانال ریشه پیچیده ای دارند. ارتباطی بین نحوه اتصال ریشه ها و وجود یا عدم وجود کانال مزیوباکال دوم وجود نداشت.

شیوع اختلالات عضلانی-اسکلتی و عوامل خطر مرتبط با آن در دندانپزشکان عمومی شهر ساری در سال 1398
مقدمه: دندانپزشکان در حرفهی خود نسبت به بسیاری از عوامل آسیبزا مانند وضعیت غیرطبیعی یا حالت قرارگیری نامناسب بدن در حین کار، در معرض خطر هستند. این عوامل سبب میشوند تا مشکلات و اختلالات اسکلتی – عضلانی (Musculoskeletal Disorders, MSDs) در میان آنها از شیوع بالایی برخوردار باشد. هدف از این مطالعه، ارزیابی شیوع MSDs و عوامل خطرساز مرتبط با آن در میان دندانپزشکان عمومی در شهر ساری در سال 1398 بود.مواد و روشها: در این مطالعه، 90 نفر از دندانپزشکان بوسیلهی پرسشنامهی استاندارد Nordic مورد ارزیابی قرار گرفتند. سپس اطلاعات بدست آمده توسط نرم افزار SPSS V.24 و آزمونهای آماری Chi-square و Multivariate regression model تجزیه و تحلیل شدند. مقدار Ρ-value کمتر از 05/0 معنیدار در نظر گرفته شد.یافتهها: از بین 90 نفر شرکت کننده، 6/64% مرد و 6/35% زن بودند و 6/75 % دارای MSDs بودند. بیشترین شیوع ناراحتیها به ترتیب در گردن (3/43%)، شانهها (0/40%)، فوقانی پشت (4/24%)، تحتانی پشت (2/22%) و مچ دست (7/17%) بوده است. علاوه بر این، 8/38% و 2/12% به ترتیب از دردهای متوسط و شدید رنج میبردند. هیچگونه ارتباط آماری معنیداری میان عوامل خطر مورد بررسی در این مطالعه با MSDs مشاهده نشد (05/0<Ρ-value). نتیجه گیری: در مطالعه حاضر، درد ناحیه گردن و شانه از شیوع بالایی در میان دندانپزشکان برخوردار بود. بهبود وضعیت بدن هنگام کارکردن و نقش متخصصین فیزیوتراپی در ارائهی آموزشهای صحیح ارگونومی واجرای دقیق این دستورالعملها توسط دندانپزشکان، جهت حفظ سلامت بدن ضروری به نظر میرسد.

بررسی رابطه بین اضطراب، افسردگی و استرس (پرسشنامه استانداردDASS21 ) با شاخص های پوسیدگی و پریودنتال در نوجوانان 15 ساله ساکن شهر بندرعباس در سال تحصیلی 97-1396
مقدمه: در این مطالعه به بررسی رابطه بین اضطراب، افسردگی و استرس با شاخص های پوسیدگی و بیماری های پریودنتال در نوجوانان 15 ساله ساکن شهر بندرعباس در سال تحصیلی 97-1396 پرداخته شد.مواد و روش ها: این تحقیق، از دسته مطالعات توصیفی- تحلیلی بود که جامعه آماری آن را کلیه دانش آموزان 15 ساله(478 نفر) شهر بندرعباس تشکیل می دادند. برای جمع آوری داده های بالینی از دو نفر برای معاینه استفاده شد. جهت اندازه گیری شاخص های استرس، اضطراب و افسردگی نیز از پرسشنامه DASS21 استفاده شد که توسط خود دانش آموزان تکمیل گردید. برای تحلیل داده ها از آمار توصیفی و همچنین تست کای دو و ANOVA استفاده گردید. یافته ها: در این مطالعه، 257 نفر (8/52 %) دانش آموز پسر و 230 نفر (2/47 %) دختر بودند. میزان اشتغال پدران دانش آموزان شرکت کننده، 445 نفر (3/91 %) بود. همچنین 162 نفر (4/33 %) از پدران و 167 نفر (3/34 %) از مادران دارای تحصیلات دانشگاهی بودند. نتایج نشان دهنده تفاوت معنادار بین سازه های اضطراب و افسردگی با شاخص بهداشت دهانی ساده بود. عمده دانش آموزان از لحاظ روانی در گروه متوسط طبقه بندی گردیدند که این تفاوت از لحاظ آماری نیز معنادار بود (001/0 > P).نتیجه گیری: بر اساس نتایج این مطالعه مشخص گردید بین سازه های تشکیل دهنده DASS21 و شاخص های مرتبط با سلامت دهان (پریودنتال، پوسیدگی) ارتباط وجود داشت و دانش آموزان با وضعیت روحی و روانی مناسب تر از وضعیت سلامت دهان بهتری برخوردار بودند.

مقدمه: با توجه به اینکه مولرهای اول یکی از اولین دندان هایی هستند که زودتر از بقیه دندان ها از دست می روند، بازسازی آن ها با ایمپلنت های دندانی شایع است. با توجه به میزان فضای مزیودیستالی باقیمانده، یک ایمپلنت با قطر پهن/متوسط یا دو ایمپلنت با قطر متوسط/باریک در این ناحیه استفاده می شود. با این حال، وقتی دو ایمپلنت بسیار نزدیک به یکدیگر قرار دارند، روند قالبگیری می تواند پیچیده شود.گزارش تکنیک: بیمار با دو ایمپلنت با قطر باریک در ناحیه مولر اول راست مندیبل جهت بازسازی پروتزی مراجعه نمود. با توجه به نزدیکی ایمپلنت ها و پهنای کوپینگ های قالبگیری، بستن همزمان دو کوپینگ جهت قالبگیری معمول ممکن نبود. در نتیجه، تصمیم گرفته شد بعد از قالبگیری دو ایمپلنت، تهیه کست ها و انتخاب اباتمنت ها بصورت جداگانه، روند آلترد کست دنبال شود. تهیه ایندکس برای کست نهایی از طریق دو روش اسپلینت کردن اباتمنت های نهایی یا کوپینگ های رزینی در داخل دهان ممکن است.نتیجه گیری: انتخاب روش قالبگیری مناسب در صورت ایده ال نبودن شرایط قالبگیری نقش مهمی در دستیابی به نتیجه درمان پروتزی موفق دارد. استفاده از روش قابگیری آلترد کست در موارد وجود دو ایمپلنت نزدیک بهم در ناحیه مولر برای موفقیت درمان کمک کننده است.
