بررسی تأثیر آموزش به روش بازخورد محور برشاخص های بهداشت دهان، خودکارآمدی و رفتارهای خود مراقبتی بهداشت دهان و دندان در دانش آموزان دختر دبیرستانی حاشیه شهر زاهدان در سال 1398
مقدمه:آموزش بهداشت دهان ودندان در خدمات اولیه بهداشتی و غربالگری کمک کننده می باشد. لذا انتخاب شیوه آموزشی موثر جهت انتقال دانش و مهارت های بهداشتی حائز اهمیت می باشد. با توجه به توان بالقوه آموزش بازخورد محور در تغییر رفتار، مطالعه حاضر با هدف تأثیر این شیوه آموزشی بر شاخص های بهداشت دهان، خودکارآمدی و رفتارهای خود مراقبتی انجام شد. موادوروش ها: این مطالعهنیمهتجربیبرروی دانش آموزهای دختر دبیرستانیدر سال 1398 انجام شد. 70 دانش آموز بهروش تصادفی انتخاب و سپس به طور تصادفی در دو گروه مداخله و کنترل تقسیم شدند. در گروه مداخله، برنامه آموزشی در 4 جلسه آموزش فردی بر مبنای بازخورد محور ارائه شد. داده ها در دو مرحله با استفاده از فرم اطلاعات فردی،پرسشنامه رفتارهای خودمراقبتی، پرسشنامه خودکارامدی بهداشت دهان و دندان،شاخص PBIجهت بررسی میزان التهاب لثه و شاخص Quigely Hein برای بررسی پلاک دندانی جمع آوری شد و با استفاده از آمار توصیفیو آزمون های تحلیلی تی مستقل و تی زوجی با استفاده از نرم افزار SPSS با ویرایش 22 انجام شد. یافته ها: دوگروهکنترلومداخلهازنظر متغیرهای فردی تفاوت معنی داری نداشتند.مقایسه میانگین نمرات پلاک دندانی و خونریزی لثه، تفاوت معنی داری را بین دوگروه نشان داد (001/0>P). آزمون تی زوجی نشان داد که گروه مداخله با بهبود وضعیت شاخص ها و گروه کنترل با افزایش معنی دار نمره خونریزی لثه )002/0P=) و پلاک دندانی (001/0>P) همراه بوده است.میانگیننمرات کل رفتارهای خود مراقبتی )001/0>(P و خودکارآمدی (001/0(P= در پایان مداخله در گروه مداخله افزایش معنی داری داشت. نتیجه گیری:آموزش بازخورد محور در بهبود شاخص های بهداشت دهان، خودمراقبتی و خودکارآمدی دانش آموزان موثر بود. پیشنهاد می شود از این مدل به عنوان رویکردی جامعهنگر، جهت بهبود رفتارهای خود مراقبتی و شاخص های بهداشت دهان استفاده شود.

بررسی اثر ضدمیکروبی و ضدبیوفیلم محلول نانونقره در رابطه با میکروارگانیسم های پلاک دندانی در شرایط آزمایشگاهی
مقدمه: ذراتنانونقره به سبب اثرات ضدمیکروبی یون نقره، پتانسیل کاربرد به عنوان عامل ضدمیکروبی را دارا می باشد. هدف از مطالعه ی آزمایشگاهی حاضر، ارزیابی خصوصیات ضد باکتریایی و ضد بیوفیلم نانونقره در برابر دو میکروارگانیسم پلاک دندانی، استرپتوکوکوس موتانس و اگریگاتیباکتراکتینومیست کومیتنس (Aa) بود. مواد و روش ها: ابتدا تست هاله عدم رشد در محیط Brain heart infusion agarبا روش Cup plate انجام شد. سپس حداقل غلظت ممانعت کننده از رشد (MIC) و حداقل غلظت کشنده ی باکتری (MBC)با استفاده از روش میکرودیلوشن تعیین شد. کینتیک مرگ باکتری با استفاده ازTime-Kill Test در زمان های مختلف 30، 60، 120 ثانیه و 5 دقیقه تعیین شد. هم چنین اثر این دو ماده در ممانعت از تشکیل بیوفیلم دو سویه باکتریایی با استفاده از روش Tissue Culture Plate (TCP) مورد بررسی قرار گرفت. تجزیه و تحلیل داده ها با آزمون های t مستقل و من-ویتنی انجام شد. یافتهها: مقایسه عملکرد ضد میکروبی دو ماده براساس آزمون tنشان داد که در غلظت μg/ml 200 نانوسیلور و μg/ml 120 کلرهگزیدین هر دو ماده اثر مشابهی روی استرپتوکوکوس موتانس دارند و در مورد اگریگاتیباکتراکتینومیست کومیتنس، کلرهگزیدین موثرتر بوده است )0001/0(P<. MIC و MBC ذرات نانونقره برای استرپتوکوکوس موتانس به ترتیب µg/ml 9/3 و 9/3 و برای اگریگاتیباکتراکتینومیست کومیتنس µg/ml5/6 و 01/13بدست آمد. ارزیابی کینتیک مرگ نشان داد که در هر دو باکتری مورد مطالعه، عامل ضد میکروبی قادر به کاهش سریع جمعیت میکروارگانیسم های زنده به میزان 6 سیکل لگاریتمی بعد از 30 ثانیه تماس با عامل ضدمیکروبی بود.بررسی های آماری نشان داد درصد کاهش بیوفیلم برای باکتری استرپتوکوکوس موتانس و اگریگاتیباکتراکتینومیست کومیتنس در گروه مواجهه با نانوسیلور نسبت به کلرهگزیدین تفاوت معنی داری نداشت )05/0(P>. نتیجه گیری: نتایج مطالعه ما نشان داد که نانونقره اثرات آنتی باکتریال معناداری با شروع اثر سریع بر میکروارگانیسم های پلاک دندانی دارد. هم چنین، باعث کاهش تشکیل بیوفیلم هر دو گونه باکتری مورد مطالعه در شرایط آزمایشگاهی می شود.

مقایسه فراوانی شکایتها، نوع و شدت حوادث استرسزا در بیماران مبتلا به درد آتیپیک صورت و درد با منشأ مشخص در مراجعه کنندگان به دانشکده دندانپزشکی زاهدان
مقدمه:(AFP) Atypical Facial Pain به صورت احساس درد مخاط دهان بدون هیچگونه اختلال مخاطی قابل مشاهده، در حین معاینات کلینیکی مشخص می شود. پزشکانی که با درد آتیپیک صورت آشنا نیستند علائم بیماران را جدی نمی گیرند و این حس را یک حس غیر واقعی می دانند، این عدم توجه پزشکان نه تنها موجب افزایش اضطراب بیمار بلکه باعث افزایش حس درد بیماران نیز می شود. مواد و روش ها: در این مطالعه، تعداد 47 نفر از بیماران مبتلا به AFP پس از کسب شرایط ورود به مطالعه از نظر سن و جنس با گروه شاهد هماهنگ شدند و پرسشنامه های هولمزراهه و فرم ثبت شکایت ها را به منظور مقایسه فراوانی شکایت ها، نوع و شدت حوادث استرس زا تکمیل نمودند. سپس اطلاعات توسط آزمونهای آماری تی (t)، کای دو، من ویتنی، رگرسیون چندگانه و لوجستیک مورد تجزیه و تحلیل قرار گرفت. همچنین سطح معناداری آماری 05/0 نظر گرفته شد. یافته ها: در این مطالعه دو گروه مورد و شاهد مورد مطالعه قرار گرفتند. بررسی نوع شکایت بیماران در گروه مورد نشان داد که در 9/80 درصد انواع دردهای غیردندانی عامل بروز درد هستند. آزمون من ویتنی نشان داد که اختلاف معنی داری در شدت درد بین دو گروه شاهد و مورد وجود نداشت (251/0P=) مدت زمان شکایت ها بین بیماران مبتلا به درد آتیپیک صورت و گروه شاهد از لحاظ آماری تفاوت معنی داری داشت (004/0P=)مقایسه حوادث استرس زا نشان دادکه در گروه مورد در 7/12 درصد فوت همسر یا اعضای خانوده عامل استرس بود. نتایج آزمون تینشان داد شدت حوادث استرس زا در دو گروه از لحاظ آماری تفاوت معنی داری داشت. (032/0P=) نتیجه گیری: براساس یافته ها، فراوانی شکایتها، نوع و شدت حوادث استرس زا در بیماران به مبتلا درد آتیپیک صورت با افراد سالم مراجعه کننده به کلینیک دانشکده دندانپزشکی زاهدان از لحاظ آماری تفاوت معنی داری داشت.

بررسی سازگاری نسجی آلیاژهای بیس متال و نابل روی فیبروبلاست لثه ی انسانی
مقدمه:از تمام آلیاژهای دندانی عناصری به حفره دهان آزاد می شود که می تواند تأثیرات منفی بر سلولهای بدن داشته باشد. هدف از این مطالعه بررسی تأثیر آلیاژهای بیس متال و نابل بر بقای فیبروبلاست های لثه انسان بود. مواد و روش ها: فیبروبلاست های بدست آمده از لثه سالم انسان حین جراحی افزایش طول تاج، کشت داده شد. در پاساژ سوم، فیبروبلاست ها در معرض غلظتهای مختلف دو نوع آلیاژ بیس متال(وراباند 2، کامند) و دو نوع آلیاژ نابل(دگوباند 4، پال کرمیت 2) قرار گرفتند. برای تکرارپذیری نتایج از هر ماده حداقل شش نمونه تهیه شد (n=18) و پس از گذشت 9 و 15 روز میزان بقای فیبروبلاستها با روش MTT مورد بررسی قرار گرفت. دادهها با آزمون ANOVA و نرم افزار SPSS ویرایش 20 مورد تجزیه و تحلیل آماری قرار گرفتند. یافته ها: هم نوع آلیاژ، هم غلظت آلیاژ و هم مدت زمان، بر درصد سلولهای باقیمانده مؤثر بود. حداکثر و حداقل سلول های باقیمانده به ترتیب در مجاورت آلیاژهای دگوباند4 و وراباند2 بود. میانگین درصد سلول های باقیمانده در غلظت 20 به طور معناداری بیشتر از غلظت 60 و در غلظت 60 به طور معناداری بیشتر از غلظت 100 بود. میانگین درصد سلول های باقیمانده در دگوباند 4 و کامند در روز 9 به طور معناداری بیشتر از روز 15 بود. اختلاف معناداری در آلیاژ پال کرمیت 2 و وراباند 2 در روز 9 و 15 دیده نشد. نتیجهگیری: آلیاژ بیس متال وراباند 2 به طور قابل توجهی بیش از سه نوع آلیاژ دیگر بقای فیبروبلاستها را به صورت وابسته به غلظت کاهش داد. آلیاژ نابل دگوباند 4، نسبت به سه نوع آلیاژ دیگر به طور قابل توجهی کمترین اثر منفی را بر بقای فیبروبلاستهای زنده داشت. بنابراین با توجه به میانگین درصد سلولهای باقی مانده، آلیاژ وراباند 2 دارای سمیت خفیف و سه نوع آلیاژ دیگر زیست سازگارهستند.

شیوع ضایعات دهانی و ارتباط آن با ریسک فاکتورها در سالمندان آسایشگاه های شهرستان بندرعباس در سال 1397
مقدمه: با افزایش میانگین سن افراد جامعه، علاوه بر مشکلات دندانی، بیماریهای مختلفی نیز مخاط دهان سالمندان را درگیر می کند. هدف از مطالعه حاضر بررسی فراوانی ضایعات دهانی و ارتباط آن با ریسک فاکتورها در سالمندان آسایشگاه های شهر بندرعباس در سال1397 بود. مواد و روش ها:این مطالعهی توصیفی – تحلیلی، بر روی 68 سالمند ساکن آسایشگاههای شهر بندرعباس در سال 1397 انجام شد. اطلاعات جمع آوری شده در چک لیست ثبت و با استفاده از نرم افزار SPSS با ویرایش 20 و آزمونهای آماری Mann- Whitney، Chi-square و Fisher exact test مورد تجزیه و تحلیل قرار گرفت. یافته ها: از 68 سالمند مورد بررسی، 42 نفر (8/61 درصد) مرد و 26 نفر (2/38 درصد) زن بودند و میانگین سنی افراد 59/70 سال بود. ضایعات دهانی در 1/69٪ افراد مشاهده شد. بیشترین ضایعه ی مشاهده شده، پیگمانتاسیون (6/42%) و شایعترین مکان برای بروز ضایعات، لثه و ریج آلوئولار (7/31%) بود. همچنین ارتباط آماری معنی داری بین حضور ضایعات دهانی و مصرف سیگار مشاهده شد (034/0 (P-value=. نتیجه گیری:با توجه به فراوانی نسبتا بالای ضایعات دهانی در سالمندان این مطالعه، لزوم پیگیری و معاینات دهانی جدی تر این قشر جامعه توصیه می گردد.

تأثیر درمان غیرجراحی پریودنتال بر شاخص کیفیت زندگی مرتبط با سلامت دهان
مقدمه: بیماری پریودنتیت به عنوان یک معضل بهداشتی دهانی مهم با اثر بر سلامت سیستمیک و کیفیت زندگی بیماران میباشد. این مطالعه جهت بررسی تغییرات شاخص کیفیت زندگی مرتبط با سلامت دهان در بیماران مبتلا به بیماریهای پریودنتال بعد از انجام فاز درمان غیر جراحی پریودنتال طراحی گردید. مواد و روش ها: در این مطالعه کلینیکی، 60 بیمار که تنها نیازمند فاز یک درمان پریودنتال (درمان غیر جراحی) بودند، وارد مطالعه شدند. برای هر یک از بیماران پرسشنامه Oral lmpact on Daily Performance (OIDP) قبل از درمان و یک ماه بعد از درمان فاز یک غیر جراحی پریودنتال کامل گردید. نمرات شاخص اندازهگیری و مقایسه شد و ارتباط آن با سن ، جنس و نوع بیماری پریودنتال بررسی شد. یافته ها: نتایج نشان داد که کیفیت زندگی در بیماران دچار بیماری پریودنتال میتواند بعد از درمان فاز غیرجراحی پریودنتال بهبود یابد. همچنین تفاوتی بین نتایج تغییرات در کیفیت زندگی در دو گروه بیماران مورد بررسی (ژنژیویت مزمن جنرالیزه و پریودنتیت مزمن جنرالیزه خفیف) دیده نشد. از نظر جنس و سن نیز تفاوت معنیداری در میزان تغییرات در شاخص کیفیت زندگی مرتبط با سلامت دهان مشاهده نشد. نتیجه گیری: براساس نتایج بدست آمده، بیماری پریودنتال میتواند تأثیر به سزایی در کیفیت زندگی بیماران مبتلا داشته باشد و درمان غیرجراحی پریودنتال میتواند موجب بهبود شاخص کیفیت زندگی مرتبط با سلامت دهان و عملکرد روزانه این افراد گردد.

مقایسه میزان غلظت سیتوکین های التهابی TNF-α و Interleukin-1β در بزاق افراد مبتلا به ژنژیویت، پریودنتیت مزمن و افراد سالم
مقدمه: پریودنتیت یک بیماری التهابی بافت های حمایت کننده دندان است که بوسیله میکروارگانیسم ها ایجاد می شود. امروزه نقش مدیاتورهای التهابی در ایمنی ذاتی و اکتسابی در پریودنتیت نشان داده شده استو بعضی از آنها نقش های مهمی در پاتوژنز پریودنتیت بازی می کنند. هدف از این مطالعه، تعیین و مقایسه غلظت سیتوکین های التهابی TNF-αو IL-1ß در بزاق افراد مبتلا به ژنژیویت،پریودنتیت مزمن و افراد سالم بود. مواد و روش ها:در این مطالعه توصیفی تحلیلی، نمونه بزاق غیرتحریکی 45 فرد شامل 15 فرد مبتلا به پریودنتیت، 15 فرد مبتلا به ژنژیویت و 15 فرد سالم جمع آوری شد. سپس میزان TNF-αو IL-1ß توسط دستگاه ELISA Reader مشخص شد. داده ها با آزمون های های آماری ANOVA و Tukey و ضریب همبستگی Pearson تجزیه و تحلیل شدند. یافته ها:میانگین غلظت IL-1ß و TNF-α بین سه گروه اختلاف معنادار داشت (001/0p <). به این صورت که میانگین غلظت TNF-α در افراد سالم به طور معناداری کمتر از گروه مبتلا به ژنژیویت و در گروه مبتلا به ژنژیویت کمتر از گروه مبتلا به پریودنتیت مزمن بود (001/0p <). میانگین غلظت اینترلوکین1 بتا در افراد سالم به طور معناداری کمتر از گروه مبتلا به پریودنتیت مزمن (001/0p <) و در گروه مبتلا به پریودنتیت مزمن کمتر از گروه مبتلا به ژنژیویت بود (001/0p =). بین غلظت IL-1ß و TNF-α در افراد مبتلا به ژنژیویت (03/0=p و 354/0=r) و نیز در کل افراد مورد مطالعه (001/0p < و 561/0=r) ارتباط مستقیم و معنادار وجود داشت. نتیجه گیری: سطح IL-1ß و TNF-α در بزاق، در شرایط بیماری پریودنتال، نسبت به شرایط سالم افزایش می یابد.

بررسی آگاهی دانشجویان دندان پزشکی عمومی و دستیاران تخصصی دانشگاه علوم پزشکی بابل نسبت به کاربرد توموگرافی کامپیوتری با اشعه مخروطی (CBCT)
مقدمه: توموگرافی کامپیوتری با اشعه مخروطی CBCT)) یک روش جدید تصویربرداری است که در آن از یک پرتو مخروطی شکل استفاده می شود. این تکنیک به طور گسترده ای در حیطه ی رادیولوژی دندانپزشکی مورد پذیرش قرار گرفته است. CBCT توسط بسیاری از دندانپزشکان به عنوان روش تصویربرداری منتخب در کنار سایر روش های متداول برای ایمپلنت های دندانی، ارتودنسی و اندو مورد استفاده قرار می گیرد. هدف از این مطالعه ارزیابی آگاهی دانشجویان دندانپزشکی دانشگاه علوم پزشکی بابل نسبت به کاربردهای CBCT در دندانپزشکی بود. مواد و روش ها: 189 نفر از دانشجویان دندانپزشکی عمومی و 19 نفر از دستیاران تخصصی در این مطالعه شرکت کردند. در این تحقیق از پرسشنامه ی 16 سوالی به عنوان ابزار اصلی جهت ارزیابی سطح آگاهی دانشجویان از کاربرد توموگرافی کامپیوتری با اشعه مخروطی (CBCT) استفاده شد. روایی پرسشنامه با استفاده از منابع، مقالات معتبر علمی و با ارایه و نظر خواهی از متخصصین و اساتید دندانپزشکی و کارشناسان مربوطه تایید گردید و پایایی با استفاده از آزمون مجدد ضریب آلفای کرونباخ (75/0) تعیین گردید. اصلاحات لازم در پرسشنامه به عمل آمد و در نهایت نتایج بدست آمده به وسیله ی نرم افزار SPSS و آزمون های t و ANOVA مورد تجزیه و تحلیل قرار گرفت. یافته ها: بنابر نتایج این مطالعه، سطح آگاهی دستیاران تخصصی از گروه دانشجویان عمومی بیشتر بوده است. همچنین بین سطح آگاهی دانشجویان با مدت زمان تحصیل در دانشگاه تفاوت معنی داری وجود داشت و با بالا رفتن سال تحصیلی، آگاهی هم بالا می رود. نتیجه گیری: نتایج مطالعه ی حاضر نشان داد دانشجویان دندانپزشکی دانشگاه علوم پزشکی بابل، دارای آگاهی نسبتاً متوسطی در زمینه ی کاربرد توموگرافی کامپیوتری با اشعه ی مخروطی (CBCT) بودند. به نظر می رسد برای رسیدن به سطح مطلوبی از آگاهی دانشجویان در زمینه ی CBCT باید آموزش های لازم در قالب های مختلف مانند کارگاه و سمینار داده شود.

بررسی میزان آلفاآمیلاز بزاق در افراد مبتلا به هرپس راجعه لب
مقدمه: هرپس راجعه یکی از زخم های دهانی شایع است که بصورت دردناک، بیشتر روی لب ها ظاهر می شود.از طرفی آلفاآمیلاز بزاق، پروتئین ترشحی است که در شرایط پراسترس سطح آن افزایش پیدا می کند. این مطالعه با هدف بررسی میزان آلفاآمیلاز بزاق در افراد مبتلا به هرپس راجعه لب صورت گرفت. مواد و روش ها: در این مطالعه ی مورد- شاهدی، بزاق غیرتحریکی 60 فرد مراجعه کننده (30 فرد مبتلا به هرپس لب و 30 فرد بعنوان شاهد) به بخش بیماری های دهان دانشکده دندانپزشکی زاهدان جمع آوری شد. سطح آلفاآمیلاز بزاق به روش اسپکتروفتومتری اندازه گیری شد و اطلاعات به دست آمده توسط نرم افزار آماری SPSS با ویرایش 19 و آزمون t-test تجزیه و تحلیل گردید.05/0>P معنادار در نظر گرفته شد. یافته ها: میانگین سطح آلفاآمیلاز تام بزاق در افراد مبتلا به هرپس راجعه لب 05/192±68/459 IU/L و در افراد سالم 51/166±29/290 IU/L بود که این اختلاف از لحاظ آماری معنادار بود. )001/0(P= نتیجه گیری: سطح آنزیم آلفاآمیلاز در دوره ی عود هرپس لب به طور معناداری بیشتر از دوره ی بهبودی و گروه شاهد بود که احتمال وجود ارتباط بین میزان آلفاآمیلاز بزاق و هرپس لب را نشان می دهد.

درمان استئونکروز فکی ناشی از دارو با استفاده از فیبرین غنی از پلاکت (PRF): گزارش یک مورد و مروری بر متون
مقدمه: هدف از مطالعه حاضر، گزارش درمان موفقیت آمیز استئونکروز مربوط به دارو در فکین (MRONJ) با استفاده از فیبرین غنی از پلاکت (PRF) به عنوان یک تسریع کننده بهبود زخم در بیمار تحت درمان با بیس فسفونات تزریقی که سابقه کشیدن دندان داشته، بود. گزارش مورد: بیمار خانمی 65 ساله با شکایت از درد، تورم و عفونت در ناحیه سمت راست فک بالا با سابقه کشیدن دندان به بخش جراحی دهان، فک و صورت دانشکده دندانپزشکی مشهد مراجعه کرد. نمایان شدن استخوان به همراه عفونت و درد در ناحیه دندان خارج شده، حدود دو هفته بعد از خارج سازی دندان پرمولر دوم فک بالا سمت راست، آغاز و بیشتر از 8 هفته در دهان بیمار باقی مانده بود که تشخیص MRONJ داده شد. با استفاده از 10 سی سی خون بیمار PRF تهیه شد. استخوان نکروز با احتیاط خارج گردید. سپس در محل جراحی PRF آماده شده، قرار داده و فلپ به صورت بدون کشش بخیه شد. در فالوآپ 6 ماهه هیچگونه عارضه ای مشاهده نشد و تمام علائم بیماری پایان یافت. نتیجه گیری: با توجه به نتیجه مطالعه ما و سایر مطالعات مرور شده، استفاده از PRF بعد از جراحی می تواند اثر بسیار مفیدی در بهبود و ترمیم زخم و ساکت استخوان در بیماری MRONJ داشته باشد. PRF نسبت به درمان های جایگزین، دارای تکنیک ساده تر و ارزانتری می باشد و می توان از این روش به عنوان روشی نوین در درمان MRONJ سود برد.
