چطور سرعت و سهولت در انتخاب کلیدواژه برای مقالات دندانپزشکی را افزایش دهیم؟
داشتن سرعت در انجام کارها یکی از مولفه های مهم بهینه سازی زمان است. البته این در صورتی مفید است که کیفیت انجام کار نیز حفظ شود. این مقاله با معرفی ابزارهای نوین به نویسندگان مقالات دندانپزشکی کمک می کند تا با انتخاب سریع کلیدواژه، ضمن حفظ کیفیت آن، صرفه جویی بیشتری در زمان نگارش مقاله خود داشته باشند.

بررسی تأثیر دوره های توانمندسازی اعضای هیات علمی دانشگاه علوم پزشکی مشهد بر رضایتمندی و دانش آنان در زمینه مهارتهای تدریس و ارزشیابی
مقدمه: این مطالعه با هدف بررسی تأثیر دوره های توانمندسازی اعضای هیات علمی دانشگاه علوم پزشکی مشهد بر رضایتمندی و دانش آنان در زمینه مهارتهای تدریس و ارزشیابی مبتنی بر مدل کرک پاتریک انجام شده است. مواد و روش ها: در این پژوهش مقطعی، اعضای هیات علمی علوم پایه و بالینی که در کارگاههای توانمند سازی در حیطه آموزشی در سال 1396 (دوره هشتم و نهم) شرکت کرده بودند مورد بررسی قرار گرفتند. جهت ارزشیابی کارگاههای مذکور بر اساس سطح اول مدل کرک پاتریک، که سطح واکنشی می باشد، از یک پرسشنامه محقق ساختهاستفاده شد. جهت ارزشیابی برنامه در سطح دوم مدل کرک پاتریک، که سطح یادگیری می باشد از آزمون های پره تست و پست تست، بلافاصله قبل و بعد از برگزاری کارگاه استفاده شد. یافته ها: در مورد کارگاههای توانمند سازی دوره نهم، بیشترین میانگین رضایت اعضای هیات علمی علوم بالینی، مربوط به کارگاه تدوین طرح دوره بود. به علاوه، بیشترین میانگین رضایت اعضای هیات علمی علوم پایه از برگزاری کارگاهها، مربوط به کارگاه روشهای تدریس دانشجو محور بوده است. در مورد کارگاههای توانمند سازی دوره هشتم، بیشترین میانگین رضایت اعضای هیات علمی علوم بالینی از برگزاری کارگاهها، مربوط به کارگاه روشهای نوین سنجش بالینی بوده است. بیشترین میانگین رضایت اعضای هیات علمی علوم پایه از برگزاری کارگاهها مربوط به مبانی آموزش مجازی و تکنولوژیهای نوین در آموزش بوده است. نمره آزمون اعضای هیات علمی هم در گروه علوم پایه و هم در گروه علوم بالینی، پس از برگزاری کارگاههای توانمند سازی دوره هشتم و نهم به طور معناداری افزایش یافته بود. نتیجهگیری: کارگاههای توانمند سازی اعضای هیات علمی توانسته است سبب بهبود یادگیری آنان در زمینه مهارتهای تدریس و ارزشیابی شود. رضایت اساتید بسته به نوع کارگاه متفاوت بوده است.

بررسی استحکام باند برشی دو نوع گلاس آینومر Fuji II LC و Ketac N100 به عاج حساسیت زدایی شده با لیزر و خمیر CPP-ACP
مقدمه: حساسیت دندانی یک درد تیز کوتاه است؛ که میتواند در اثر پاسخ عاج اکسپوز شده به محرکهای لمسی و شیمیایی ایجاد شود. راهکار اصلی در درمان حساسیت عاجی بستن توبولهای عاجی توسط رسوبات کریستالی دردهانه توبول یا ذوب کردن هیدروکسی آپاتیت عاجی است. هدف از این مطالعه بررسی تاثیر حساسیت زداهای عاجی بر میزان استحکام باند برشی مواد ترمیمی گلاس آینومر به عاج بود. مواد و روشها: 60 دندان پر مولرسالم انسان جهت مطالعۀ حاضرانتخاب شدند. سطح دندان تحت برش اکلوزالیmm 2 زیر DEJ تاج قرارگرفت.نمونهها به سه گروه 20 تائی تقسیم شدند؛ گروه اول: عاج دست نخورده، گروه دوم: تابش لیزر دیود کم توان با شدت j/cm² 7/3، زمان5 دقیقه و طول موج µm 66/0بر سطح عاجی، گروه سوم: کاربرد خمیر ضدحساسیت CPP-ACp روی عاج آکلوزالی. هرگروه به دو زیر گروه 10 تائی تقسیم شد.زیرگروههای اول با Fuji II LC بر مرکز سطح عاجی قرار گرفته در مولد پلاستیکی و زیرگروههای دوم با نانو آینومر Ketac N100 پر شد. سپس تست برشی نمونهها انجام شد و اندازهگیری استحکام باند با استفاده از آزمون ANOVA و Tukey صورت گرفت. یافتهها: در گروههای Fuji II LC تفاوت معنیداری بین میانگین استحکام باند برشی در دو گروه درمان و گروه کنترل (Laser، Control، CCP-ACp ) مشاهده نشد. (P=0.086) ولی در گروه N100Ketac میانگین استحکام باند برشی گروه درمان شده با لیزر به طور معنیداری از دو زیرگروه کنترل (027/0P=) و CPP-ACp بیشتر بود (035/0P=). نتیجه گیری: در درمان CPP-ACp و کنترل، گلاس آینومر Fuji II LC نتایج بهتری نسبت به Ketac N100 داشت. با این حال، در گروههای درمان شده با لیزر تفاوت معنیداری بین دو نوع گلاس آینومر وجود نداشت.

بررسی تأثیر جنس Post-Core بر استحکام شکست در بازسازی تاج دندان های درمان ریشه شده
چکیده مقدمه: دندان های درمان ریشه شده در برابر نیروهای فانکشنال بیشتر مستعد به شکست می باشند. در این موارد اغلب درمان پست و کور تجویز می گردد. از سوی دیگر استفاده از رستوریشن های تمام سرامیک با بهبود خواص آن ها در بازسازی دندان های خلفی گسترش یافته است. هدف از این مطالعه، بررسی استحکام شکست چهار سیستم پست و کور در دندان های درمان ریشه شده پره مولر با روکش تمام سرامیک بود. مواد و روش ها: ۴۸ عدد دندان پرمولر اول مندیبل به طور تصادفی در چهار گروه تقسیم شدند. پس از انجام درمان ریشه استاندارد، به ترتیب در گروه اول تا چهارم از پست و کور ریختگی (Ni-Cr)، پست گلاس فایبر و کورکامپوزیت، پست و کور ریختگی Non-Precious Gold و پست و کور زیرکونیایی Custom Milled استفاده گردید. کلیه نمونه ها با روکش تمام سرامیکی با کوپینگ زیرکونیا بازسازی شده و تحت بارگذاری با دستگاه Zwick قرار گرفتند. میزان نیروی قابل تحمل توسط نمونه ها تا نقطه شکست اندازه گیری شده و نوع شکست نیزدر هر گروه مورد بررسی قرار گرفت.نتایج با آزمون های آماری آنالیز واریانس و آزمون اختلاف معنی دار توکیارزیابی شدند (a=0.05). یافتهها: میانگین استحکام شکست در چهار گروه مورد مطالعه به ترتیب 270±59/780، 323±70 / 915،۳۰۷±۲۴/۹۶۳ و 220±34/435 نیوتن گزارش شد. این تفاوت در گروه پست و کور زیرکونیا معنی دار بود (001/0P=). نتیجه گیری: این مطالعه نشان داد که از بین سیستم های پست و کور مورد استفاده در بازسازی پروتزی پرمولرهای درمان ریشه شده با روکش تمام سرامیک، پست و کورهایCustom Milled زیرکونیا به طور معنی داری استحکام شکست پایین تری نسبت به سایر گروه ها داشته و شکستگی های غیر قابل ترمیم با این پست ها شایع تراست.

تأثیر مداخله آموزشی بر آگاهی، عملکرد و نگرش پزشکان عمومی در رابطه با ضایعات زخمی حفره دهان
مقدمه: بهداشت دهان یکی از مؤلفه های اصلی سلامت عمومی و کیفیت زندگی افراد است و با تشخیص به موقع بیماریهای دهان، پیشآگهی و درمان آنها تسریع میشود. لذا این مطالعه با هدف مداخله و ارزیابی آگاهی، عملکرد و نگرش پزشکان عمومی در ارتباط با ضایعات زخمی حفره دهان طراحی و اجرا شد. مواد و روشها:این مطالعه به صورت مداخلهای نیمه تجربی با برگزاری دورههای بازآموزی روی 75 نفر از پزشکان عمومی شرکت کننده در مرکز توسعه آموزش پزشکی (EDC) دانشگاه علوم پزشکی اصفهان انجام شد. ابزار گردآوری اطلاعات شامل یک پرسشنامه استاندارد شده در دو قسمت بود؛ قسمت اول شامل اطلاعات جمعیت شناختی و قسمت دوم، شامل سه بخش آگاهی، عملکرد و نگرش درباره زخم حفره دهان بود.تجزیه و تحلیل دادهها با استفاده از نسخه 20 نرمافزار SPSS، t-test و ضریب همبستگی پیرسون انجام شد. یافته ها:میانگین نمره آگاهی آکادمیک، آگاهی کلینیکی، نگرش و عملکرد پزشکان بین سه زمان پس از مداخله اختلاف معناداری نشان داد(001/0 P≤). نمره نگرش و عملکرد با سن (001/0 P=و 03/0P=) و مدت طبابت (002/0 P= و 02/0P=) رابطه مستقیم اما با تعداد معاینه بیمار در روز (61/0P=و 29/0P=) رابطه معنادار نداشت.همچنین نمره آگاهی آکادمیک و کلینیکی با سن (92/0 P=و 59/0P=)، مدت طبابت (93/0 P=و 92/0P=) و تعداد معاینه بیمار در روز (77/0 P=و 85/0P=)رابطه معنادار نداشت. نتیجه گیری: با توجه به تاثیر مثبت مداخله در این مطالعه، ارتقای آگاهی پزشکان و پرسنل خدمات بهداشتی-درمانی با استفاده از برنامههای آموزشی مدون و ترکیبی ضمن خدمت و مدلهای تغییر رفتار به عنوان یکی از عوامل موثر در پیشگیری از ضایعات حفره دهان ضروری میباشد.

ارزیابی استرس ریج باقیمانده و جابجایی مخاط زیر اوردنچر متکی بر دو ایمپلنت فک پایین به وسیله روش آنالیز اجزای محدود
مقدمه: توزیع مناسب استرس در نواحی بی دندانی در اوردنچرهای متکی بر دندان فک پایین ضروری است. این توزیع استرس تحت نیروهای متفاوت به کمک الگوی عناصر محدود مورد بررسی قرار گرفت. هدف از این مطالعه، ارزیابی استرس ریج باقیمانده و جابجایی مخاط زیر اوردنچر متکی بر دو ایمپلنت فک پایین به وسیله روش آنالیز اجزای محدود بود. مواد و روش ها: مدل دیجیتالی فک پایین با دو ایمپلنت (ITIبا قطر 1/4 و طول 12 میلیمتر) در نواحی کانین و اوردنچر متکی بر آن توسط نرم افزار آباکوس شبیه سازی شد. نیروی عمودی بر دندان آسیای اول سمت چپ بتدریج از 0 تا 50 نیوتن وارد گردید. توزیع استرس حاصله مورد ارزیابی قرار گرفت. یافته ها: تحت نیروهای اکلوزالی اندک، جابجایی مخاطی و تجمع استرس استخوان کورتیکال ریج بی دندان، در سمت غیر کارگر دیده شد و بتدریج با افزایش نیرو به سمت کارگر منتقل شد. تجمع استرس استخوان کورتیکال بطور عمده در اطراف ایمپلنت و با میزان کمتر در ریج خلفی مشاهده گردید. جابجایی مخاطی اولیه و به میزان بیشتر در ناحیه بی دندانی خلفی و بخصوص روی ریج مایلوهایوئید رویت شد. نتیجه گیری: در این مطالعه روش آنالیز اجزای محدود، مشاهده شد که جابجایی مخاط نواحی خلفی به خصوص روی ریج مایلوهایوئید و کرست ریج بی دندان نسبت به نواحی قدامی اطراف ایمپلنت بیشتر است؛ بنابراین این نواحی باید با دقت رلیف گردد و فشارها تا جای ممکن به طرف باکال شلف هدایت شود.

بررسی شیوع و علل خارج سازی پیچ و پلاک در جراحی های ارتوگناتیک دانشکده دندانپزشکی مشهد در طی سالهای 97-1395
مقدمه: هدف این مطالعه، تعیین میزان و دلایل خارج سازی مینی پلیت ها در بیماران جراحی ارتوگناتیک طی سالهای 1397-1395 در دانشکده دندانپزشکی مشهد بوده است. مواد و روش ها: در این مطالعه مقطعی، همه بیمارانی که به بخش جراحی دانشکده دندانپزشکی مشهد جهت جراحی ارتوگناتیک در طی سال های 95 تا 97 مراجعه کرده بودند، وارد مطالعه شدند. خارج سازی پیچ و پلاک بعد جراحی، علل مربوط به خارج سازی پلیتها، سن و جنس و نوع عمل ارتوگناتیک تعیین شد. در نهایت نتایج با استفاده از نرم افزار SPSS با ویرایش 21 آنالیز آماری گردید. یافته ها: در این مطالعه خارج سازی پیچ و پلیت و عوامل مرتبط با آن در 69 بیمار جراحی ارتوگناتیک شده بین سالهای 95 تا 97، مورد ارزیابی قرار گرفتند. شیوع خارج سازی پیچ و پلیت در این بیماران 7 نفر (14/10 درصد) بود. عفونت و اکسپوژر پلیت شایعترین علت خارج سازی بود. خارج سازی پیچ و پلیت در فک پایین بیشتر از فک بالا بود ولی معنی داری نبود (05/0P<). همچنین با مصرف مشروبات الکلی ارتباط معنی داری داشت (05/0P<). نتیجه گیری: با توجه به میزان کم خارج سازی پلیتهای فک و صورت در بیماران جراحی ارتوگناتیک (14/10 درصد) ، نیازی به خارج سازی مینی پلیت های تیتانیومی بدون علامت و مشکل نبوده و فقط وقتی لازم است پلیت ها خارج شوند که اندیکاسیون کلینیکی و علامت واضح داشته باشند. عفونت و اکسپوژر پلیت شایعترین علت خارج سازی پیچ و پلیت بعد جراحی ارتوگناتیک بود.

بررسی تاثیر افزایش مدت زمان جراحی دندان عقل مندیبل بر بروز درای ساکت
مقدمه: درای ساکت شایعترین عارضه پس از جراحی دندان عقل نهفته است. در مشاهدات بالینی، حفره دندان کشیده شده خالی از لخته خون بوده و استخوان، قابل رویت است. مدت زمان جراحی از عوامل تاثیرگذار بر بروز این ضایعه معرفی شده است. اما بررسی مطالعات صورت گرفته پیرامون تاثیر افزایش طول مدت جراحی دندان عقل مندیبل بر بروز درای ساکت، به نتایج دقیقی منجر نشده است. لذا هدف از این مطالعه، بررسی تاثیر افزایش طول مدت جراحی دندان عقل مندیبل بر روی درای ساکت بود. مواد و روش ها: در این مطالعه 227 نفر بیمار در رده سنی 17 تا 30 سال که جهت جراحی دندان عقل مندیبل به بخش جراحی دانشکده دندان پزشکی مشهد مراجعه کرده بودند، مورد بررسی قرار گرفتند. درجه سختی دندان های جراحی شده بر اساس رفرنس پیترسون بین 4 تا 8 در نظر گرفته شد. در روزهای 2و4و7 پس از جراحی، میزان درد و بروز درای ساکت مورد بررسی قرار گرفت. یافته ها: میانگین درد در روز دوم بعد جراحی 08/1±72/4، و در روز چهارم 26/1±57/1 و در روز هفتم 11/1±29/0 بود. اختلاف درد بین زمانهای مختلف اندازه گیری شده معنی دار بود. (001/0P<)در روز دوم بعد جراحی درای ساکتی مشاهده نشد اما در روز چهارم 5 مورد درای ساکت و در روز هفتم نیز فقط همان 5 مورد درای ساکت مشاهده گردید. فراوانی درای ساکت بین سه زمان بعد جراحی، دارای تفاوت معنی دار بود. (007/0P=)، افزایش سختی جراحی، مدت زمان جراحی نیز بطور معنی داری افزایش یافت، همچنین با افزایش مدت زمان جراحی، مقدار درد در روزهای دوم و هفتم بعد جراحی نیز بطور معنی داری بیشتر شد؛ حال آنکه این مقدار در روز چهارم تنها به مقدار خیلی جزئی افزایش یافت و مقدار همبستگی آن معنی دار نبود. نتیجه گیری: افزایش طول مدت زمان جراحی دندان عقل مندیبل با بروز درای ساکت ارتباط معناداری دارد.

مقایسه اثر اندواکتیویتور و فایل XP-Endo Finisher در پاکسازی کانال های آلوده به کاندیدا آلبیکنس : یک مطالعه آزمایشگاهی
مقدمه: دستیابی به موفقیت قابل پیش بینی در درمان ریشه نیازمند پاکسازی موثر کانال است. پاکسازی مکانیکی و شیمیایی، اکثر باکتری های پاتوژن را حذف می کند اما گاهی به دلیل وجود پیچیدگی هایی مانند کانال های فرعی و محدودیت دسترسی شستشو دهنده ها به کانال، پاکسازی دشوار می شود. مطالعات متعددی حضور قارچ ها در عفونت های اندودانتیک و نقش آنها در اتیولوژی ضایعات پری رادیکولار را نشان داده اند. مطالعه حاضر با هدف مقایسه اثر اندواکتیویتور و فایل XP‐Endo Finisher با روش معمول در پاکسازی کانالهای آلوده به کاندیدا آلبیکنس انجام شد.
مواد و روش ها: 56 دندان سانترال ماگزیلای تک ریشه و تک کانال انتخاب شدند. برای ایجاد عفونت، کاندیدا آلبیکنس در محیط سابور دکستروز آگار کشت داده شد و به مدت یک روز انکوبه شد. پس از دوره انکوباسیون، دندانها به صورت تصادفی به 4 گروه آزمایش 14 تایی تقسیم شدند. درگروه اول از فایل XP‐Endo Finisher، در گروه دوم از اندواکتیویتور، در گروه سوم از هیپوکلریت سدیم 25/5 درصد و درگروه چهارم از نرمال سالین جهت شستشوی کانال استفاده شد. در نهایت پس از اتمام مدت انکوباسیون، تعداد کلنی های رشد نموده بر روی محیط کشت شمارش شده و بر اساس واحد CFU/ml گزارش شد.
یافته ها: نتایج نشان داد میانگین تعداد کلونی کاندیدا آلبیکنس پس از کاربرد XP‐Endo Finisher، اندواکتیویتور و هیپوکلریت سدیم در کلیه نمونه ها صفر بود. از طرفی میانگین تعداد کلونیها پس از کاربرد نرمال سالین CFU/ml 08/171723 بود بنابر نتایج این مطالعه، فراوانی کلونیهای کاندیدا آلبیکنس پس از کاربرد XP‐Endo Finisher، اندواکتیویتور و هیپوکلریت سدیم 25/5 درصد به طور معنی داری کمتر از نرمال سالین بود و هر سه برای شستشوی کانال مناسب ارزیابی می گردند.
نتیجه گیری: استفاده از XP‐Endo Finisher، اندواکتیویتور و هیپوکلریت سدیم 25/5 درصد برای شستشوی کانال با هدف حذف کاندیدا آلبیکنس پیشنهاد می گردد.

بررسی تغییرات ضربان قلب و درصد اشباع اکسیژن خون حین جراحی دندان مولر سوم فک پایین
مقدمه:جراحیدندانمولر سومیکیاز جراحی هایشایعفکو صورتاستکهدر مواقعیمیتواند دشوار و زمانبر باشد. از اینرو کنترلعلائمحیاتیبیمار در اینجراحیضرورریبهنظر میرسد. هدف از این مطالعه بررسی تغییرات ضربان قلب و درصد اشباع اکسیژن خون در حین جراحی دندان مولر سوم فک پایین بود.
مواد و روشها:اینیکمطالعهمقطعی توصیفی بود که بر روی 50 نفر از افرادی که برای جراحی دندان مولر سوم فک پایین بهبخش جراحی دانشکده دندانپزشکی دانشگاه علوم پزشکی شیراز مراجعه کرده بودند، انجام شد. تعداد ضربان قلب و درصد اشباع اکسیژن خون در مراحل مختلفجراحی، شامل هنگام نشستن بیمار در اتاق انتظار، نشستن بیمار بر روی صندلی دندانپزشکی، تزریق ماده بی حسی، انجام برش جراحی، برداشتن استخوان، درآوردندندان مولر سوم، بخیه زدن و در پایان عمل جراحی اندازه گیری شدند.جهت تحلیل آماری از آزمون Repeated measure ANOVA و Independent sample t-test در سطح خطای 05/0 استفاده شد.
یافته ها:از کل بیماران، تعداد 28 نفر مذکر (56%) و 22 نفر مونث (46%) بودند. میانگین سنی افراد در این مطالعه 8/8±12/30 سالبود. میانگین کلی تعداد ضربان قلب بیماران در هشت مرحله اندازه گیری شده 3/5±52/80 ضربه در دقیقه و درصد کلی اشباع اکسیژن خون در هشت مرحله 7/0±22/97%به دست آمد. بین تعداد ضربان قلببیماران در مراحل مختلف جراحی(001/0P<) و همچنین درصد اشباع اکسیژن (001/0P<) از لحاظ آماری رابطه معنی دار وجود داشت.
نتیجه گیری: این مطالعه نشان داد که بیشترین افزایش تعداد ضربان قلب بیمارانیکه تحت جراحی دندان مولر سوم فک پایین قرار می گیرند، در مرحله تزریق ماده بی حسی می باشد و در حین این عمل جراحی، درصد اشباع اکسیژن خون در محدوده نرمال باقی می ماند.

مدیریت درمان دندانپزشکی اکتودرمال دیسپلازی هایپوهیدروتیک، گزارش یک مورد
مقدمه: اکتودرمال دیسپلازی یک بیماری نادر ژنتیک است که دو یا تعداد بیشتری ساختار اکتودرمال مانند مو، ناخن، دندان، غدد عرق و .... را درگیر می کند. درگیری های سیستمیک این بیماران شامل هایپوتریکوز ، پوست خشک هایپوپیگمانته و هایپرترمی است. درگیری های داخل دهانی این بیماران شامل الیگودنشیا و دندان های مخروطی ست. .هایپودنشیا، الیگودنشیا و ابنورمالیتی های متعدد در شکل دندان ها(دندان مخروطی) و تاخیر رویش دندانی در هر دو سیستم دندانی شیری و دائمی در این بیماری یافت می شود. گزارش مورد: بیمار پسر 8 ساله مبتلا به دیسپلازی اکتودرمال نوع هایپوهیدروتیک بدون بیماری سیستمیک و سابقه ی بستری شدن در بیمارستان و مورد مصرف داروی خاص مراجعه کرد. شکایت اصلی بیمار عدم رویش دندان های قدامی و آبسه دندان شیری و خروج چرک بود. بنا بر گزارش والدین، کودک با ظاهر دهان خود مشکل داشته و در مدرسه از خندیدن و صحبت کردن امتناع می کرد. همچنین به دلیل کمبود بزاق، سختی در جویدن و بلع گزارش شد. نیازهای زیبایی کودک ارزیابی شد. با توجه به عدم ثبات ابعاد استخوان تا سن 18 سالگی، امکان قرار دادن ایمپلنت دندانی برای او وجود نداشت. نتیجه گیری: در این مطالعه هدف از درمان، بازسازی فکی با روشی مقرون به صرفه و کارآمد از یک سو و ارائه درمانی جامع برای پیشگیری از بیماری های دندانی و درمان های ترمیمی از سوی دیگر بود. درمان های زیبایی انجام شده برای کودک سبب ارتقای اعتماد به نفس کودک شده و در فالوآپ های سه ماهه، 6 ماهه و یک ساله، والدین کودک، پیشرفت عملکرد تحصیلی کودک و ارتقای روابط اجتماعی او را در مدرسه و خانواده گزارش کردند.
