بررسی تظاهرات دهانی در بیماران دارای بیماری کلیوی مراحل آخر تحت همودیالیز
مقدمه:نارسایی مزمن کلیه بر اثر تخریب برگشت ناپذیر نفرون ها به وجود می آید، که در آن توانایی بدن در حفظ تعادل آب و الکترولیت ها از بین رفته و در نتیجه اورمی ایجاد می شود.تظاهرات دهانی مختلفی مرتبط با نارسایی کلیوی می باشند و می توانند اثرات قابل ملاحظه ای بر کیفیت زندگی این افراد داشته باشند. با تشخیص و بررسی شیوع این ضایعات دهانی در بیماران کلیوی می توان برنامه ها و مداخلات مناسب به منظور رفع علائم دهانی ضایعات طراحی نمود. لذا در این مطالعه به بررسی شیوع علائم دهانی در بیماران کلیوی مراحل آخر و تحت همودیالیز (ESRD) پرداختیم. موادوروش ها:اینمطالعهازنوعمقطعی بود. جامعهموردمطالعهبیماران ESRD تحتهمودیالیزمراجعهکنندهبهبیمارستانامامرضا)ع( کرمانشاهبود. قبلازورودبیمارانبهمطالعهومعاینهحفرهدهان،ازبیمارانرضایتنامهاخذگردید. برایاینمنظورپرسشنامهایتهیهشدکهشاملعلائمونشانههایدهانیبود.علائمونشانههایپسازمعاینهکاملحفرهدهاننیز توسطمحققثبتگردید. داده ها با استفاده از آزمون آماری Chi-Square بررسی شد. (05/0α=) یافته ها:در مطالعه حاضر، 120نفر حضور داشتند که 74نفر (7/61%)مرد و 46نفر (3/38%) زن بودند. شایع ترین علتهای زمینه ای ابتلا به نارسایی کلیوی، فشار خون (8/35%) و دیابت (2/24%) بودند.شایع ترین تظاهرات دهانی شامل خشکی دهان (7/66%) بوی اورمیک (8/45%) زبان باردار (5/37%) و پتی شی، پورپورا و اکیموز (35%)بودند. افزایش سن با افزایش خشکی دهان در ارتباط بود. همچنین بین دیابت و خشکی دهان (04/0=P)، پتی شی، پورپورا و اکیموز (022/0=P) و تغییر مزه (007/0P=)از لحاظ آماری ارتباط معنی داریوجود داشت. نتیجه گیری:نارسایی مزمن کلیه باعث تغییراتی در حفره دهان از جمله خشکی دهان، زبان باردار، بوی اورمیک، پتی شی، پورپورا، اکیموز و تغییر مزه می شود. بنابراین می بایست بیماران مبتلا به ESRD از لحاظ تظاهرات دهانی عنوان شده مورد معاینه دوره ای قرار بگیرند.

تعیین اثر خمیردندان حاوی پروپولیس بر پلاک دندانی در دانشجویان دانشکده دندانپزشکی زاهدان
مقدمه:کنترلپلاکهمیشهازمسائلمهمدندانپزشکیبودهوروشهاوموادگوناگونیبهاینمنظورارائهشدهاست. یکی از این مواد پروپولیس استکهخاصیتآنتیباکتریالوضدالتهابدارد. هدفازاینمطالعهتعیین اثر خمیردندان حاوی پروپولیس بر پلاک دندانی در دانشجویان دانشکده دندانپزشکی زاهدانبود. مواد و روشها:اینکارآزماییبالینی دوسو کورباالگویمتقاطع بر روی ۳۰ نفر از دانشجویان دندانپزشکی صورت گرفت. در ابتدا تمامی دانشجویان مورد معاینه لثه قرار گرفتند و برای هرکدام در حالت Base line شاخصهای لثهای و پلاک تهیه شد و جرم دندانها برداشته شد. دانشجویان به دو گروه تقسیم شدند. به گروه اول خمیردندان حاوی پروپولیس و به گروه دوم خمیردندان بدون پروپولیس داده شد. بعد از مدت ۲ هفته استفاده از خمیردندانها شاخصهای لثه و پلاک برای تمامی افراد مورد مطالعه تهیه شد. پس از یک هفته Wash out شاخص های لثه و پلاک مجدد اندازه گیری شد. اینبار به گروه اول خمیردندان بدون پروپولیس و به گروه دوم خمیردندان حاوی پروپولیس داده شد و بعد از دو هفته استفاده شاخص های لثه و پلاک اندازه گیری شد. تحلیلدادههابااستفادهازنرمافزار SPSS 22 وآزمون t مستقلوزوجیانجامشد. یافتهها: میزان شاخص پلاک در هر دو نوع خمیردندان کاهش داشت اما در خمیردندان حاوی پروپولیس کاهش بیشتری مشاهده شد(002/0P=) و میزان شاخص لثهای در هر دو گروه کاهش پیدا کرد اما این کاهش میزان شاخص لثهای در گروهی که خمیردندان حاوی پروپولیس استفاده کرده بودند معنیدار بود (001/0P<). نتیجهگیری: پروپولیس در خمیردندان می تواند در کاهش میزان شاخص های لثه و پلاک، نقش مؤثری داشته باشد.

مقایسه تغییرات ابعادی سیلر NanoZnO با سیلرهای پالپ دنت و AH26
مقدمه:در درمان ریشه، سیل کردن سیستم کانال ریشه پس از مرحله پاکسازی و شکل دهی، ضروری و واجب است. با توجه به معیارهای Grossman ثبات ابعادی از جمله ملزومات یک سیلر است. هدف این مطالعه، مقایسه سیلر نانو زینک اکساید اوژنول با پالپ دنت و AH26 از نظر ثبات ابعادی بود. مواد و روش ها:در این مطالعه برای بررسی تغییرات حجم سیلر ضمن ستینگ، سه گروه ده تایی از سیلر های AH26، پالپ دنت،NanoZOE درون پیپت مدرج با دقت 001/0 که به شکلU خم شده بود، قرار گرفت. حجم معینی از سیلر درون مجموعه حاوی آب و متیلن بلو، درون انکوباتور با دمای 37 درجه قرار گرفت و سپس نمونه ها در ساعت اول و دوم پس از اختلاط، هر 15 دقیقه و در ساعت سوم هر 20 دقیقه و پس از 24 ساعت، یک هفته و 30روز خوانده شد. یافته ها:به طور کلی سیلر AH26 دارای کمترین تغییرات در طول زمان بوده است. سیلر پالپ دنت، تا زمان 90 دقیقه تغییرات ابعادی کمی داشته، اما از زمان 90 دقیقه تا یک هفته بیشترین تغییرات ابعادی را داشته است. سیلر نانو تا زمان 90 دقیقه بیشترین تغییرات را داشت و سپس تا زمان یک هفته، دارای تغییرات ابعادی بیشتر از سیلر AH26 و کمتر از سیلر پالپ دنت بوده است. پس از زمان یک ماه، تغییرات ابعادی بین سه سیلر معنی دار نبوده است. نتیجه گیری:درپایان زمان یک ماه، تغییرات ابعادی نمونه ها در گروه AH26 کمترین و در گروه پالپ دنت بیشترین مقدار را داشت، اما در کل بین سه سیلر تفاوت معنی داری وجود نداشت.

مقایسه اثر ضد میکروبی کامپوزیت حاوی نانواکسید روی و مس بر علیه استرپتوکوکوس موتانس
مقدمه:انقباض حین پلیمریزاسیون باعث ایجاد شکاف بین کامپوزیت و لبه دندان می شود، که در طول زمان باعث ورود باکتری ها، پوسیدگی ثانویه و در نهایت شکست درمان میشود.بنظر میرسد اضافه کردننانوذرات به کامپوزیت میتواند در کاهش تعداد و عملکرد میکروارگانیسمها مؤثر باشد. هدفاین مطالعه، مقایسه اثر ضد میکروبی کامپوزیتهای حاوی نانوذرات اکسید روی و مس بر روی گونه میکروبی استرپتوکوکوس موتانس بود. مواد و روش ها: نانوذرات اکسید مس و روی در غلظت های 05/0، 1/0 و 5/0 درصد با کامپوزیت GCمخلوط و 490 دیسک کامپوزیتی در هفت گروه شامل 70 نمونه تهیه شد. محلول های رقیق شده ازاسترپتوکوک موتانس (ATCC 35668) تهیهو 1میلی لیتر از آن روی دیسک ها قرار گرفت. دیسک ها با محیط کشت مایع در دمای 37 درجه به مدت 8 ساعت انکوبه شدند. سپس 1 میلیمتر سرم شستشوی حاصله از دیسک ها در دمای 37 درجه به مدت 24 ساعت بر روی محیط بلادآگار حاوی خون گوسفند کشت داده شد و با شمارش (Colony count) باکتری های مستقر شده روی هر دیسک، شمارش باکتری ها به دست آمد. داده ها پس از جمع آوری وارد SPSSبا ویرایش 17 شد و با استفاده از آزمون کروسکال- والیس و آزمون دان، مورد مقایسه آماری قرار گرفت. سطح معنی داری 05/0=α در نظر گرفته شد. یافته ها: این تحقیق نشان داد که نانوذرات اکسید مس و روی در غلظت های 1/0و 5/0 درصد در زمان های 15 و 30 روز در مقایسه با گروه کنترل باعث کاهش معنیدار تعداد باکتریها شد (001/0P=) و سایر گروه های مورد بررسی، تفاوت آماری معنیداری با گروه کنترل نداشتند (1P=). نتیجه گیری: نانوذرات اکسید روی 5/0 درصد در ترکیب کامپوزیت، بیشترین و هر دو نانوذرات اکسیدمس و روی در غلظت 05/0 درصد کمترین میزان خاصیت ضدمیکروبی را از خود نشان دادند.

تأثیر فسفات بافر سالین روی استحکام باند Mineral Trioxide Aggregate در حضور آلودگی خونی در پرفوراسیون های شبیه سازی شده ناحیه فورکا
مقدمه: استحکام باند به عاج موادی که برای ترمیم پرفوراسیونها استفاده می شوند از فاکتورهای تأثیرگذار روی توانایی سیل این مواد و موفقیت ترمیم پرفوراسیونها می باشد. هدف این مطالعه بررسی تأثیر Phosphate Buffered Saline (PBS) بر استحکام باند پوش اوت(WMTA) White Mineral Trioxide Aggregate در حضور آلودگی خون در پرفوراسیونهای شبیه سازی شده ناحیه فورکا بود. مواد و روش ها: این مطالعه در مرکز تحقیقات بیماری های لثه و دندان دانشکده دندانپزشکی تبریز با کد اخلاقی به شمارهIR.TBZMED.REC.B1396.819در سال 1396 انجام گرفت. در60 مولر اول مندیبل انسان، پرفوراسیون ناحیه فورکا به قطر 3/1 و ارتفاع 2 میلیمتر ایجاد و سپس نمونه ها به صورت تصادفی به چهار گروه 15 تایی تقسیم شدند. در گروه های 3 و 1 مداخله دیگری بعد از ایجاد پرفوراسیون و شستشو با نرمال سالین انجام نشد. در گروه های 4 و 2، آلودگی با خون در پرفوراسیون ناحیه فورکا ایجاد شد. سپس در تمام نمونه ها یک عدد Gel tamp در زیر ناحیه پرفوره قرار گرفت.در تمام دندانها WMTA در ناحیه پرفوره قرار گرفت. یک تکه پنبه مرطوب، آغشته به نرمال سالین در گروه های 4 و 1 به رویMTA گذاشته شد. در گروه های 3 و 2 یک تکه پنبه مرطوب، آغشته به PBS به رویMTA قرار داده شد. بعد از یک هفته، تست اینسترون توسط دستگاه یونیورسال انجام شد. با توجه به نرمال بودن توزیع داده ها از تست Two way ANOVA جهت آنالیز آماری استفاده گردید و 05/0P< به عنوان معنی دار تلقی گردید. یافته ها: در حضور و عدم حضور آلودگی خونی میانگین استحکام باند پوش اوت WMTA در مجاورت نرمال سالین به صورت معنی دار بیشتر ازPBS بود (03/0P=). آلودگی خون به صورت معنی داری باعث کاهش استحکام باند گردید. (04/0P=) نتیجه گیری: حضور فسفات بافر سالین در پرفوراسیون فورکا، تأثیر مثبتی روی افزایش استحکام باند WMTA ندارد.

مقایسه اثر دنتین باندینگ نسل 5 و 6 بر جلوگیری از تغییر رنگ دندان ناشی از MTA سفید
مقدمه:MTA می تواند سبب تغییر رنگ تاجی بعد از درمان های رژنراتیو اندودانتیک شود. جلوگیری از تغییر رنگ ناشی از MTA در ناحیه زیبایی اهمیت بسزایی برای بیماران و کلینیسین ها دارد. هدف از این مطالعه، ارزیابی تأثیر انواع مختلف باندینگ های عاجی در پیشگیری از تغییر رنگ بوجود آمده در اثر MTA سفید بود. مواد و روش ها: از 36 دندان سنترال اینسایزور ماگزیلا استفاده شد. بعد از انجام پروسه اندودانتیک، دندانها به سه گروه تقسیم شدند. در گروه اول و دوم قبل از کاربرد MTA سفید در حفره دسترسی به ترتیب Adper Single Bond(SB) (نسل 5) و Clearfil SE Bond(CSE)(نسل 6) استفاده شد. در گروه سوم، حفره دسترسی به MTA سفید آغشته شد. حفره دسترسی در این سه گروه سیل شد. رنگ نمونه ها توسط اسپکتروفتومتر ارزیابی شد. ∆Eبعد از دو ماه نگهداری در انکوباتور و سرم فیزیولوژی اندازه گیری شد. داده ها توسط تست آماری کلموگراف-اسمیرنوف، آنالیز واریانس یک طرفه و توکی، آنالیز شدند. سطح معناداری 05/0 درنظر گرفته شد. یافته ها: تفاوت بین گروههای اول و دوم معنادار نبود (546/0p =) در صورتی که ∆E بین گروه های اول و سوم و همچنین دوم و سوم معنادار بود (001/0p <). نتیجه گیری: باندینگهای عاجی می توانند از تغییر رنگ ناشی از MTA جلوگیری کنند. اگرچه تفاوت بین نسلهای 5 و 6 باندینگها از لحاظ آماری معنادار نبود، نمونه های آماده شده با ادهزیو نسل 6 تغییر رنگ تاجی کمتری نشان دادند.

بررسی انحراف کاسپی در ترمیم های مزیواکلوزودیستالی با کاربرد کامپوزیت بالک فیل و ادهزیو یونیورسال
مقدمه: امروزه جهت کاهش مراحل و حساسیت ترمیم های کامپوزیتی، کامپوزیت های بالک فیل و ادهزیوهای یونیورسال عرضه شده اند.این پژوهش با هدف تعیین انحراف کاسپی در ترمیم های مزیواکلوزودیستالی (MOD) با کاربرد کامپوزیت بالک فیل و ادهزیو یونیورسال انجام شد. مواد و روش ها: 58 دندان پرمولر کشیده شده در 4 گروه تقسیم بندی و بصورت بالکی و لایه ای با استفاده از کامپوزیت Filtek Bulkfill و ادهزیوهای G-Premio و Adper Single Bond ترمیم شدند. ابتدا در هر دندان یک حفره استاندارد مزیواکلوزودیستالی تهیه شد به گونه ای که عرض باکس پروگزیمالی دو سوم عرض باکولینگوالی، عرض ایسموس اکلوزالی نصف عرض باکولینگوالی، فاصله کف ایسموس اکلوزالی از نوک کاسپ پالاتال 5/3 میلی متر و کف باکس پروگزیمالی 1 میلی متر بالای CEJ بود. قبل و بعد از ترمیم، فاصله اینترکاسپال هر دندان به ترتیب در فواصل زمانی 0،30 ،60 و180 ثانیه بعد از لایت کیور، توسط میکرومتر دیجیتال اندازه گیری شد و میانگین مقادیر حاصل محاسبه گردید. سپس مقدار انحراف کاسپی با کسر نمودن عدد بعد از ترمیم از عدد قبل از ترمیم به دست آمد. پس از انتقال مقادیر حاصله به نرم افزار SPSS با توجه به نرمال بودن نتایج، آنالیز واریانس یکطرفه انجام و سطح معنی داری 05/0 در نظر گرفته شد. یافته ها: نتایج پژوهش نشان داد بین مقادیر انحراف کاسپی گروه های مطالعه، اختلاف معنا داری وجود داشت. در مقایسه ی دو به دوی گروهها با استفاده از آزمون Tukey تنها بین گروه ترمیم شده به روش لایه ای و کاربرد ادهزیو G-Premioبا گروه ترمیم شده به روش بالکی و کاربرد ادهزیو G-Premio تفاوت معنی داری مشاهده شد. نتیجه گیری: با توجه به نتایج مطالعه حاضر بنظر میرسدمی توان با کاربرد بالکی کامپوزیتهای بالک فیل و استفاده از ادهزیوهای توتال اچ و یونیورسال، زمان کارکرد کلینیکی را کاهش داد.

بررسی ارتباط بین حس انسجام در مادران و کیفیت زندگی مرتبط با سلامت دهان و دندان در کودکان 5-3 ساله مراجعهکننده به کلینیک طوبی در شهر ساری، سال 1398
مقدمه:حس انسجام (SOC) عاملی تأثیر گذار بر رفتارهای مرتبط با سلامت میباشد. SOC مادر میتواند سلامتی و به دنبال آن کیفیت زندگی مرتبط با سلامت دهان و دندان (OHRQoL)Oral Health Related Quality of Life را در فرزند وی تحت تأثیر قرار دهد. هدف از مطالعهی حاضر، بررسی ارتباط بین SOC مادران و کیفیت زندگی مرتبط با سلامت دهان و دندان در کودکان 5-3 ساله در شهر ساری بوده است. مواد و روش ها:تعداد 190 کودک به همراه مادرانشان از میان افراد مراجعه کننده به کلینیک طوبی در شهر ساری انتخاب شدند. SOC مادران بوسیلهی پرسشنامهی SOC-13 آنتونوفسکی و کیفیت زندگی مرتبط با سلامت دهان و دندان نیز با استفاده از پرسشنامهی (ECOHIS) Early Childhood Oral Health Impact Scale ارزیابی شد. از شاخص dmft نیز جهت بررسی وضعیت پوسیدگی دندان کودکان استفاده شد. مقدار p –value کمتر از 05/0، معنیدار در نظر گرفته شد. یافته ها: در این مطالعه میانگین نمرات شاخص های SOC برابر 9/4±13/57 ، ECOHIS برابر 4/0±8/13 و dmft برابر 3/1±4/7 تعیین شده است. شاخص dmft با SOC همبستگی منفی (001/0>p –value، 453/0-=r) و با ECOHIS همبستگی مثبت (001/0>p –value،535/0=r) داشته است، به صورتی که با افزایش میزان dmft در کودکان، نمرات ECOHIS نیز افزایش، اما نمرات SOC در مادران کودکان کاهش پیدا کرده است. همچنین ECOHIS با SOC همبستگی منفی داشته است (001/0>p –value،347/0-=r)، به تعبیر دیگر، با افزایش نمرات SOC در مادران، از نمرات کیفیت زندگی نیز کاسته شده است. نتیجهگیری:SOC مادران، وقوع پوسیدگی دندانی و کیفیت زندگی را در کودکان آنها به طور چشمگیری تحت تأثیر قرار میدهد. به گونهای که با افزایش SOC در مادران، میتوان شاهد بهبود کیفیت زندگی مرتبط با سلامت دهان و دندان نیز بود.

بررسی اثر ضدمیکروبی گیاه (توربید) Daphne Oleoides روی باکتریهای جدا شده از پلاک دندانی
مقدمه: حفره دهانی محیط مطلوبی برای رشد و کلونیزاسیون طیف گسترده ای از باکتری ها می باشد. پلاک یا بیوفیلم دندانی به مجموعه ای متنوع از باکتری ها اطلاق می شود که بر روی سطح دندان یافت می شوند. هدف از این مطالعه، بررسی اثر ضدمیکروبی گیاه توربید Daphne Oleoidesروی باکتریهای جدا شده از پلاک دندانی بود. مواد و روش ها:این مطالعه مقطعی، پس از جمع آوری پلاک های دندانی، جداسازی باکتری های موجود در نمونه ها، شناسایی باکتری ها توسط تست های بیوشیمیایی و واکنش زنجیره پلیمراز (Polymerase Chain Reaction) صورت گرفت. برگ و ساقه گیاه D.Oleoidesدر فصل های رویش جمع آوری و در سایه خشک شد. عصاره آبی، الکلی و هیدروالکلی این گیاه با روش عصارهگیری کلاسیک استخراج شد. سپس اثرات ضد میکروبی آن با روش انتشار دیسک و حداقل غلظت مهار کنندگی مورد بررسی قرار گرفت. تجزیه و تحلیل داده ها با استفاده ازآزمون کروسکال-والیس و Dunn انجام شد (05/0a=). یافته ها:در پلاک های دندانی جمع آوری شده، حضور باکتری های استافیلوکوکوس اورئوس، استافیلوکوکوس اپیدرمیدیس، سودوموناس آئروجینوزا و استرپتوکوکوس موتانس به اثبات رسید. عصاره الکلی بیشترین فعالیت ضدمیکروبی خود را علیه باکتری استرپتوکوکوس موتانس با قطر هاله عدم رشد 05/1±55/20 میلی متر و کمترین غلظت کشندگی 19/0 میلی گرم بر میلی لیتر نشان داد. از سویی کمترین توان ضدمیکروبی عصاره الکلی علیه باکتری استافیلوکوکوس اپیدرمیدیس با قطر هاله عدم رشد 5/0±4/7 میلی متر و غلظت 5/12 میلی گرم بر میلی لیتر نشان داده شد(05/0P<). نتیجه گیری: نتایجتحقیقحاضرنشاندادکهعصاره الکلیگیاه D.Oleoides تواناییاستخراجترکیباتآنتیباکتریاییبر ضد باکتری های ایجاد کننده عفونت های دهانی را دارد.

گزارش یک مورد تشخیص ترومبوسیتوپنی ایمونولوژیک با شکایت اصلی عدم تطابق پروتز دندانی
مقدمه: ترومبوسیتوپنی ایمونولوژیک (Immune thrombocytopenia) به واسطه واکنش خودایمنی بر علیه پلاکت و تخریب آن شناخته می شود. تظاهرات در بیماران علامت دار شامل کبودی و خونریزی های متعدد ناشی از کاهش پلاکت می باشد. گزارشمورد: بیمارمردی 75سالهباشکایتعدم امکان استفاده از دست دندان قدیمی خود از ده روز قبل، به بخش بیماری های دهان، فکوصورتدانشکده دندانپزشکیمشهدمراجعهنمود. درمعاینه دهانی، پورپورا و اکیموز در کل کام بیمار و دو بول هموراژیک بر روی ریج مندیبل رویت شد. همچنین در معاینه بالینی روی ساق پاهای بیمار، کبودی و ماکولهای قرمز رنگ متعدد مشاهده شد. برای بیمار درخواست آزمایش کامل خون اورژانسی داده شد. باتوجه به نتیجه آزمایش بیمار مبنی بر ترومبوسیتوپنی بسیار شدید، با تشخیص احتمالی ITPیا لوسمی به بیمارستان ارجاع شد. در نهایت، تحت کورتون تراپی با دوز بالا و شیمی درمانی با ریتوکسیماب وریدی قرار گرفت و تشخیص ITPبرای بیمار مسجل شد. بیمار در طی دو فالوآپ بعدی، در طی یک سال گذشته، سلامتی خود را بازیافت. نتیجهگیری: معاینه دهانی دقیق در بیماران دارای دنچر قدیمی با مشکلات حاد عدم تطابق حائز اهمیت ویژه است. چرا که این بیماران معمولا سالمند بوده و مستعد انواع دیسکرازی های خونی هستند. در بسیاری از موارد تغییرات دهانی جزو اولین علایم بیماری های سیستمیک هستند و تشخیص صحیح می تواند جان این بیماران را به واسطه ی ارجاع به موقع جهت درمان، نجات دهد.
