بررسی مقایسه سطح هیدروکسی دی اکسی گوانوزین در بزاق بیماران با استوماتیت آفتی راجعه و افراد سالم
مقدمه: باتوجه به مطرح شدن نقش استرس اکسیداتیودر اتیوپاتوژنز استوماتیت آفتی راجعه، هدف مطالعه بررسی صدمه استرس اکسیداتیو به DNA را در این بیماری بود. محصول استرس اکسیداتیو به DNA8 – هیدروکسی داکسی گوانوزین (8-OHdG) می باشد. مواد و روشها: این مطالعه از نوع موردی-شاهدی (Case/control) می باشد. 30 بیمار مبتلا به استوماتیت آفتی راجعه و 30 فرد سالم که از نظر سن و جنس با هم همسان شده بوده اند، انتخاب شده اند. معیارهای ورود به مطالعه شامل نداشتن تاریخچه بیماری سیستمیک، نگرفتن دارو، عدم استعمال سیگار و الکل و عدم استفاده از مکمل آنتی اکسیدان در 3 ماه گذشته بود. معیار ورود به گروه مورد عود حداقل سه بار در سال آفت بود. جمع آوری بزاق به صورت غیرتحریکی و تحلیل آماری با آزمون t-student انجام شد (05/0P=). یافته ها: در این مطالعه در هر یک از دو گروه مورد و شاهد 2/63 درصد زن و 7/36 درصد مرد وجود داشت. میانگین و انحراف معیار سطح 8-هیدروکسی داکسی گوانوزین بزاق بیماران در گروه بیماران 77/12±84/38 و در افراد سالم 78/11±28/41 بود که تفاوت معنی داری نداشتند. سطوح 8 – هیدروکسی داکسی گوانوزین بزاق بیماران با استوماتیت آفتی راجعه و افراد سالم تفاوت آماری معنی داری نداشت (427/0(P=.

مقایسه میزان رضایتمندی بیماران از نگهدارنده های هاولی و فرم داده شده در خلأ
مقدمه: دو ریتینر (نگهدارنده) هاولی (Hawley) و فرم داده شده در خلا با نام تجاری Essix معمولاً برای ریتنشن بعد از درمان ارتودنسی به کار می روند. هدف از مطالعه حاضر مقایسه رضایتمندی بیماران از ریتینرهای Hawley و Essix بود. مواد و روش ها: در این مطالعه بالینی، 90 بیمار 14 تا 25 ساله که تحت درمان ارتودنسی ثابت قرار گرفته بودند و نیاز به ریتینر داشتند، انتخاب و بر اساس پروتکل ریتنشن به سه گروه تقسیم شدند. در گروه اول، نگهدارنده Hawley به صورت 4 ماه تمام وقت و سپس شبانه استفاده شد. در گروه دوم(Essix4M) بیماران از نگهدارنده Essix به صورت چهار ماه تمام وقت و سپس شبانه استفاده کردند. در گروه سوم (Essix1W) بیماران از نگهدارنده Essix به صورت یک هفته تمام وقت و سپس به صورت شبانه استفاده کردند. بررسی رضایتمندی بیماران 1 هفته و 8 ماه بعد از تحویل ریتینر توسط پرسشنامه انجام شد. داده ها با نرم افزار SPSSدر سطح معنی داری 05/0 مورد تجزیه و تحلیل قرار گرفت. یافته ها: میزان رضایت کلی بر اساس جمع امتیاز حاصل از پرسشنامه هفته اول در گروه سوم (Essix1W) بیشتر از سایر گروه ها بود، اما تفاوت میان سه گروه معنی دار نبود (885/0P=). پس از گذشت 8 ماه از تحویل پلاک میزان رضایت کلی در گروهHawley بیشتر از سایر گروه ها بود، ولی باز هم تفاوت بین گروه ها معنی دار نبود (118/0=P). نتیجه گیری: در رابطه با رضایتمندی بیماران تفاوت معنی داری بین دو نوع ریتینرHawley و Essix (با پروتکل های زمانی مختلف) مشاهده نشد. البته تفاوت های جزئی نشان دهنده محبوبیت کوتاه مدت پروتکل Essix1W و رضایت طولانی مدت بیماران از ریتینر Hawley بود.

ردیابی ویروس پاپیلومای انسانی (HPV) در آملوبلاستوما به روش مولکولی واکنش زنجیره ای پلی مراز (PCR)
مقدمه: آملوبلاستوما از شایع ترین تومورهای ادونتوژنیک می باشد. از فاکتورهای موثر در بروز این بیماری، ویروس پاپیلومای انسانی (HPV) است. مطالعه حاضر، با هدف بررسی حضور HPV در نمونه های تومور آملوبلاستومای فکین با استفاده از روش PCR انجام شد. مواد و روش ها: در این مطالعه، 77 بلوک پارافینی مربوط به آملوبلاستوما از آرشیو بخش پاتولوژی دانشکده دندانپزشکی مشهد و بیمارستان قائم مشهد جمع آوری و پس از دپارافینیزه کردن نمونه ها، DNA آنها با استفاده از Genomic DNA extraction mini YTA kitبراساس دستور کارخانه سازنده استخراج گردید. پس از انجام فرآیند PCR، حضور HPV در آنها مورد بررسی قرار گرفت. داده ها با استفاده از نرم افزار SPSSنسخه 16، آزمون کای دو و رگرسیون لوجستیک مورد تجزیه و تحلیل قرار گرفتند. یافته ها: از مجموع 77 ضایعه مورد بررسی، 20 مورد فولیکولار، 4 مورد اکانتوماتوز، 3 مورد بازال سل، 8 مورد دسموبلاستیک، 17 مورد یونی سیستیک و 25 مورد پلکسی فرم بود. در مجموع 5 مورد (5/6 درصد) از آنها از نظر HPV مثبت بود. بدین صورت که در نوع فولیکولار یک مورد، در نوع یونی سیستیک دو مورد و در نوع پلکسی فرم نیز دو مورد از نظر HPV مثبت شده بود. نتیجهگیری: با توجه به فراوانی کم ویروس پاپیلومای انسانی در ضایعات مورد بررسی، نمی توان این گونه نتیجه گرفت که این ویروس در اتیولوژی و پاتوژنز این دسته از ضایعات نقش داشته باشد و جهت اظهار نظر دقیق تر به مطالعاتبا حجم نمونه بزرگ تر نیاز است.

بررسی سلامت دهان و رفتارهای بهداشتی دهان و دندان جمعیت بالغین عشایر در سال 1395
مقدمه: عشایر گروهی از مردم با سطح اقتصاد پایین و تحصیلات اندک و دسترسی محدود به خدمات بهداشتی درمانی هستند. این مطالعه در نظر داشت سلامت دهان و رفتارهای بهداشت دهان و دندان این جمعیت خاص را ارزیابی کند. مواد و روشها: در این مطالعه میدانی، 200 نفر در سنین 18 تا 60 سال توسط نمونه گیری دو مرحله ای (خوشه ای و سپس آسان) از دهستان عشایری و چشمه لنگان شهرستان فریدون شهر در بهار و تابستان 1395 مورد بررسی قرار گرفتند. معاینات دندانپزشکی به منظور تعیین شاخصهای دندان های پرشده و کشیده شده و دارای پوسیدگی (DMFT) و شاخص پرودنتال جامعه CPI(Community Periodontal Index) انجام شد. کد 0 نشان دهنده لثه نرمال، کد 1، وجود خونریزی خودبه خودی با همراه پروب، کد 2، نشان دهنده وجود جرم، کد 3، نشان دهنده وجود پاکت 5-4 میلی متری بود. رفتارهای بهداشتی دهان و دندان و هم چنین عادات تغذیه ای از طریق چک لیست ارزیابی شد. دادهها پس از جمع آوری در نرم افزار SPSS با ویرایش 22 وارد شد و آنالیز آماری دادهها به کمک آزمونهای T-test، ANOVA و مجذور کای انجام شد (05/0=α). یافته ها: 83 نفر (5/41 درصد) مرد و 117 نفر (8/58 درصد) زنشامل 163 نفر (5/81 درصد) از دهستان عشایری و 37 نفر (5/18درصد) از دهستان چشمه لنگان بودند. 1/29 درصد (58 نفر) اصلاً دندانها را تمیز نمیکردند و تنها 3 درصد (6 نفر) هر روز مسواک میزدند. میانگین DMFT معادل 6/5±6/9 (28-0) که حدود 56 درصد سهم این شاخص مربوط به کشیدن و 40 درصد پوسیدگی بود. اختلاف معنی داری بین زن و مرد وجود نداشت ولی بین گروههای سنی اختلاف معنی داری مشاهده شد (001/0P<) . میزان DMFT در گروه سنی 34-18سال معادل 3/4±7/6، گروه سنی 51-35 سال معادل 1/5±5/12، گروه سنی 68-52 سال معادل 1/5±3/14 بود. میزان DMFT بین گروه سنی 51-35 و 68-52 سال معنی داری نبود. 2 درصد (4 نفر) کد 0، 5/29 درصد (59 نفر) کد 1، 1/61 درصد (123 نفر) کد 2 و 7 درصد (14 نفر) کد 3 داشتند. نتیجه گیری: میزان پوسیدگی در عشایر بالا و مراجعات این افراد به دندانپزشک اغلب به جهت کشیدن دندان است. درصد بالایی از افراد، خونریزی از لثه را گزارش نمودند. از نظر بی دندانی و وضعیت پریودنتال نیز در مقایسه با میانگین جامعه، وضعیت این افراد نامطلوب ارزیابی شد.

مقایسه تأثیر مهارکننده های بافتی بر فعالیت ضدمیکروبی یک شوینده ی جدید حاوی نانوذره ی نقره، هیپوکلریت سدیم و کلرهگزیدین
مقدمه: یک شوینده ی جدید حاوی نانو ذرات نقره (Im-based AgNPs) اثرات ضد میکروبی قابل قبولی نشان داده است. هدف از این مطالعه، ارزیابی مهارکنندههای بافتی مختلف (پودر عاج، ماتریکس عاج،آلبومین سرم گاوی و لیپوپلی ساکارید باکتریال) بر فعالیت ضدمیکروبی هیپوکلریت سدیم، کلرهگزیدین و یک شوینده جدید حاوی نانو ذرات نقره (Im-based AgNPs) بود. مواد و روش ها: در سری اول آزمایش ها، µL۹۵۰ از هر یک از شوینده ها با µL۵۰ از سوسپانسیون باکتری انتروکوکوس فکالیس مخلوط گردیدند و زنده ماندن باکتری ها بعد از ۱۰، ۳۰ و ۶۰ ثانیه ارزیابی شد. در سری دوم آزمایش، µL۱۰۰ از سوسپانسیون هر یک از مهارکنندهها با µL۱۰۰ از هر یک از شوینده ها مخلوط و به مدت 1 ساعت انکوبه شدند. سپس µL۱۰۰ از سوسپانسیون میکروبی به آن ها اضافه گردید و زنده ماندن باکتری ها بعد از ۱۰ ثانیه، ۱۰ و ۶۰ دقیقه بررسی شد. هر دو سری آزمایش ها سه بار تکرار شدند و تعداد کلونی ها شمارش شده و سپس لگاریتم بر مبنای ۱۰ آنها محاسبه شد. از تست Kruskal-wallis و Dunn برای آنالیز آماری استفاده شد. 05/0P< معنی دار در نظر گرفته شد. یافته ها: در سری اول آزمایش، در هر سه زمان فعالیت ضد میکروبی هیپوکلریت سدیم و کلرهگزیدین بیش تر از AgNPs و سایلین بود. (۰۵/۰˂P) در سری دوم آزمایش هر چهار مهارکننده ی بافتی، فعالیت ضد میکروبی کلرهگزیدین و هیپوکلریت سدیم را کاهش دادند. (۰۵/۰˂P) غیر از لیپوپلی ساکارید باکتریال که بر روی هیپوکلریت سدیم اثری نداشت. AgNPs مشابه با گروه کنترل منفی، رشد بالای باکتری ها را صرف نظر از وجود یا عدم وجود مهارکننده ها نشان داد. نتیجه گیری: مهارکننده های بافتی فعالیت ضد میکروبی هیپوکلریت سدیمو کلرهگزیدین را به درجات مختلفی تحت تأثیر قرار دادند. AgNPs صرف نظر از حضور یا عدم حضور مهارکننده ها اثر ضد میکروبی قابل توجهی نشان نداد.

مقدمه: با اینکه رفتار بهداشتی دهان و دندان با نگرش مرتبط است، رفتار مثبت در پی نگرش مثبت قابل بحث است. خودکارآمدی مسواک زدن، ابزار پیش بینی کننده مناسبی برای رفتار بهداشتی محسوب می شود. هدف این مطالعه بررسی توصیفی نگرش نسبت به مسواک زدن در نوجوانان اصفهان و ارتباط آن با خودکارآمدی مسواک زدنو عادت آن بود. مواد و روش ها: مطالعه از نوع مقطعی-تحلیلی و روی 400 دانش آموز 15-13 ساله مدارس دولتی شهر اصفهان صورت گرفت. نمونه گیری تصادفی دومرحله ای طبقه بندی شده انجام شد.با استفاده ازپرسشنامه های رواشده 40 گزاره ای نگرش نسبت به مسواک زدن و 6 گزاره ای خودکارآمدی مسواک زدن، این دو سازه به ترتیب با طیف 0 تا 160 و 6 تا24 سنجیده شد. پس از استخراج یافته های توصیفی وضعیت نگرش و حیطه های آن، ارتباط آن با خودکارآمدی افراد در زمینه مسواک زدن توسط آزمون های آماری من ویتنی و همبستگی اسپیرمن بررسی شد. یافته ها: در مجموع 381 پرسشنامه کامل شده، در دانش آموزان با میانگین سنی 3/0±2/14 سال و نسبت جنسیتی برابر مورد مطالعه قرار گرفت. میانگین نمره کل نگرش در جمعیت مورد مطالعه 3/13±76 بود. نمره کل نگرش (021/0P=) و حیطه مسئولیت پذیری (008/0P=) در دختران بیشتر بود.میانگین نمره خودکارآمدی 3/4±9/15 بوده و نمره کل نگرش با خودکارآمدی ارتباط معنی داری نشان نداد، اما حیطه های ارزش ها، احساسات و مسئولیت پذیری ارتباط معنی داری داشتند. ارتباط نگرش(01/0P=) و خودکارآمدی (001/0P=) با دفعات مسواک زدن معنادار بود. نتیجه گیری: وضعیت کلی نگرش به مسواک در جمعیت مورد مطالعه متوسط ارزیابی شد. با وجودی که پرسشنامه نگرش به مسواک زدن با خود گزارشی مسواک زدن ارتباط معنی دار نشان داد. اما براساس خودکارآمدی مسواک زدن در جمعیت حاضر روایی معیاری نداشت. جنسیت در نمره نگرش نسبت به مسواک زدن موثر و در خودکارآمدی بی تاثیر بود.

بررسی فراوانی و درجه سختی نهفتگی دندان های عقل پایین در بیماران مراجعه کننده به بخش جراحی دهان و فک و صورت دانشکده دندانپزشکی مشهد
مقدمه: وجود دندان های نهفته پدیده شایعی است و درجوامع مختلف، میزان شیوع و توزیع آن تنوع قابل ملاحظهای دارد. امکان بروز مشکلات در صورت عدم تشخیص به موقعو درمان صحیح دندان های نهفته عقل پایین، بیانگر اهمیت این موضوع می باشد. هدفاینمطالعه، تعییندرجه سختیدندان های عقل نهفته دربیماران مراجعه کنند هبه دانشکده دندان پزشکی مشهد بود. مواد و روش ها: دراین مطالعه توصیفی از نوع مقطعی، 743 بیمار مراجعه کننده به مرکز جراحی دانشکده دندان پزشکی مشهد از مهر تا اسفند ماه 1396 مورد بررسی قرار گرفتند. موقعیت، زاویه نهفتگی دندان و درجه سختی نهفتگی دندان های عقل پایین از طریق گرافی پانورامیک ثبت و تعیین گردید. یافته ها: از تعداد 1118 دندان نهفتهمولر سوم پایین، 456 دندان (41 درصد) در موقعیت مزیو انگولار، 292دندان (26 درصد) موقعیت افقی، 276 دندان (25 درصد) موقعیت عمودی، و 94 دندان (8 درصد) در موقعیت دیستوانگولار بودند. نهفتگی دندان در زنان شایع تر بود (58 درصد).شایع ترین درجه سختی نهفتگی مشاهده شده،نوع متوسط بود (53 درصد). ارتباط معنی داری بین جنس و درجه سختی دندان وجود نداشت. ولی ارتباط معنی داری بین جنس با زاویه نهفتگی دندان عقلبر اساس طبقه بندی Winter & Schiller وجود داشت. همچنین درجه سختی با عمق نهفتگی دندان های عقل پایین و ارتباط با راموس بر اساس طبقه بندی Pell & Gregory وجود داشت (001/0P<). نتیجه گیری: طبق نتایج این تحقیق، فراوانی دندان های نهفته با سختی متوسط و مشکل در بیماران مراجعه کننده به دانشکده ندانپزشکی مشهد نسبتاً بالا میباشد که این موضوع، اهمیت انجام غربالگری، تشخیص و درمان بهموقع را نشان می دهد.

بررسی میزان توانمندی حرفه ای و عوامل مرتبط با آن در دانشجویان دندان پزشکی با روش خود ارزیابی
مقدمه:مطالعه ی حاضر با هدف تعیین میزان توانمندی حرفه ای و عوامل مرتبط با آن در دانشجویان سال آخر دانشکده های دندان پزشکی تیپ 1، 2 و 3 کشور به روش خودارزیابی انجام شده است.. مواد و روش ها: در این مطالعه مقطعی، تعداد 131 نفر از دانشجویان سال آخر دندان پزشکی سه تیپ دانشگاه علوم پزشکی در سال 1396 مورد بررسی قرار گرفتند. ابزار مورد بررسی، چک لیست 135 آیتمی، شامل سه بخش اطلاعات فردی-اجتماعی، توانمندی حرفه ای دانشجویان بر اساس آخرین برنامه آموزشی دوره دندانپزشکی عمومی در بخش های بالینی و تعیین عوامل موثر بر توانمندی حرفه ای دانشجویان بود. جهت تجزیه و تحلیل داده ها از آزمون های آمار توصیفی و تحلیلی، ضریب اسپیرمن، کای اسکور، من ویتنی و تی تست مستقل در سطح معنی داری 05/0 استفاده گردید. یافته ها:در مقایسه بخش های مشابه در دانشکده های تیپ های 1، 2 و 3 تفاوت ها در بخش های پروتز، اندو و ارتودنسی از نظر آماری معنی دار بود(05/0P<) در کل مهارت دانشجویان بخش سلامت دهان، بیشترین وارتودنسی کمترین میزان بود. سن و میزان معدل تاثیری در مهارت دانشجویان نداشت. با توجه به ضرایب رگرسیونی در سه بخش پریودنتولوژی، اندودانتیکس و بیماری های دهان و فک و صورت، بین محل دانشکده و توانمندی دانشجویان ارتباط معنی دار بود. (05/0P<) نتیجه گیری:با توجه به میانگین امتیاز ها که نزدیک به متوسط بود، بازنگری در شیوه های آموزشی مطابق با کوریکولوم آموزشی در بخش های بالینی ضروری به نظر می رسد.

چالش های اجرای برنامه درسی دندانپزشکی عمومی از دیدگاه دانشجویان: یک مطالعه کیفی
مقدمه: بازنگری برنامه های درسی به دلایلی شامل ظهور علوم جدید، علایق شخصی اعضای جدید هیات علمی، تغییرات دموگرافیک، پیشرفت در علوم بیولوژیک و تغییرات بنیادین در سیستم ارایه خدمات سلامت، امری اجتناب ناپذیر است. از این رو بررسی دیدگاه های ذینفعان برنامه درسی می تواند راهنمای مناسبی برای برنامه ریزان آموزشی باشد. این مطالعه به منظور بررسی چالش های اجرای برنامه درسی دندانپزشکی عمومی از دیدگاه دانشجویان در سال تحصیلی 96-1395، انجام شد. مواد و روش ها: این مطالعه به صورت تحلیل محتوای کیفی مرسوم و گردآوری اطلاعات با مصاحبه های گروهی بود. نمونه گیری به صورت هدفمند و با مشارکت 10 نفر از دانشجویان دندانپزشکی عمومی در حال تحصیل با برنامه درسی اخیراً بازنگری شده دندانپزشکی عمومی، انجام شد. برای تحلیل داده ها از نرم افزار 10MAXQDA استفاده شد. یافته ها: پس از پیاده سازی مصاحبه ها و تحلیل داده ها، 104 کد، 20 طبقه فرعی و 4 طبقه اصلی شامل چالش های اجرایی (ناپختگی و بیتجربگی در اجرا، نبود برنامهریزی اجرایی و نبود هماهنگی اساتید در ارائه دروس ادغام یافته)؛ آموزشی (نبود منابع اطلاعاتی کافی برای توجیه سازی دانشجویان، زمان محدود ارائه آموزش، ناتوانی در بکارگیری اساتید متخصص برای دروس جدید، بی عدالتی در آموزش، نامناسب بودن ادغام، استفاده از رزیدنت های غیرمسلط و آموزشندیده در آموزش دانشجویان، شیوه ارائه مباحث، محتوای نامناسب در دروس جدید، رعایت نکردن پیشنیاز های آموزشی و مقدماتی، رعایت نکردن استاندارد های آموزش پزشکی توسط اساتید) ؛ جانبی و ذهنی (رزیدنت محوری و تخصصگرا بودن اساتید، نبود منبعی برای طرح مشکلات و ارائه راه حل عملی، عادت به کوریکولوم قبلی، نگرش منفی به کوریکولوم جدید) و ساختاری (تغییرات ساختار کوریکولوم، امکانات فیزیکی و کمبود نیروی هیات علمی) استخراج شد. نتیجهگیری: چالش های بیان شده توسط دانشجویان، بر لزوم تشکیل تیم بازنگری مسلط به برنامه درسی، ایجاد پیش زمینه های تغییر، فراهمسازی منابع فیزیکی و انسانی، اجرای آزمایشی برنامه درسی و پایش توسط متخصصین آموزش پزشکی، تاکید دارد.

بازسازی دندان های قدامی ماگزیلای یک بیمار دچار براکسیسم با دنتیشن سایش یافته به صورت متوسط: گزارش یک مورد
مقدمه: تشخیص قطعی بازسازی سطوح سایش یافته و همزمان کنترل فاکتورهای مسبب این سایش ها یک گزینه درمانی مناسب است. هدف این گزارش مورد، توصیف سکانس درمانی جراحی افزایش طول تاج زیبایی و بازسازی ترمیمی بیمار دچار براکسیسم با دنتیشن سایش یافته به صورت متوسط است. گزارش مورد: در این مطالعه، یک آقای 18 ساله به بخش پریودنتولوژی دانشکده ی دندانپزشکی دانشگاه علوم پزشکی تهران مراجعه کرد. طرح درمان بیمار شامل جراحی افزایش طول تاج در ناحیه ی سکستانت قدامی ماگزیلا و انجام بلیچینگ داخلی در دندان سانترال سمت چپ فک بالای بیمار و سپس بازسازی سکستانت قدامی ماگزیلا با ونیر سرامیکی(IPS e.max) بود. نتیجه گیری: در بیمار حاضر، جراحی افزایش طول تاج انجام و پس از طی دوره ی ترمیم حدود دو ماهه، باز سازی تاج انجام شد. به این ترتیب علاوه بر اینکه تجاوز به عرض بیولوژیک صورت نگرفت، تاج از دست رفته ی دندان بازسازی شده و ابعاد عمودی دندان ها اعاده می شود.
