بررسی استفاده موضعی از عسل در پیشگیری از موکوزیت ناشی از شیمی درمانی در کودکان مبتلا به لوسمی
مقدمه: موکوزیت دهانی یکی از عوارض شایع در بیماران تحت شیمی درمانی است که موجب درد، ناراحتی و عفونت می شود. مطالعه حاضر به منظور بررسی استفاده موضعی از عسل در پیشگیری از موکوزیت ناشی از شیمی درمانی در کودکان مبتلا به لوسمی انجام گرفت. مواد و روشها:این مطالعه کارآزمایی بالینی تصادفی شده یک سوکور، روی 40 بیمار 14-1 ساله که تحت شیمی درمانی قرار داشتند، انجام شد. بیماران به چهار گروه مورد (دهانشویه عسل)، کنترل مثبت (دهانشویه کلرهگزیدین)، کنترل منفی (نرمال سالین) و مداخله (دهانشویه بابونه) به تعداد مساوی از نظر سن و جنس تقسیم شدند. در هر گروه پد گازی آغشته به دهانشویه مورد نظر به مخاط سطح گونه، کام و زبان مالیده شد. طی روزهای اول، هفتم و چهاردهم پس از شیمی درمانی، دهان کودکان از نظر وجود موکوزیت بر اساس معیار WHO مورد معاینه قرار می گرفت. اطلاعات با استفاده از نرم افزار SPSS ، در سطح معنی داری 05/0 ، با استفاده از آزمون کروسکال - والیس و من ویتنی مورد آنالیز قرار گرفتند. یافته ها: میزان موکوزیت ایجاد شده در روز اول مورد بررسی در کلیه گروههای مورد بررسی صفر بود. در روز هفتم نیز در کلیه گروهها به جز نرمال سالین (30 % در حد خفیف و 30 % در حد متوسط) صفر و در روز چهاردهم نیز در کلیه گروهها به جز نرمال سالین (100% خفیف) صفر بود. نتیجه گیری: بر اساس نتایج حاضر، استفاده موضعی از عسل، میتواند به صورت مشابه با بابونه و کلرهگزیدین در پیشگیری از موکوزیت ناشی از شیمی درمانی در کودکان مبتلا به لوسمی موثر باشد.

بررسی ارتباط حجم و پنوماتیزاسیون سینوس ماگزیلاری با سن و جنس افراد
مقدمه:هدف از این مطالعه مقطعی، بررسی حجم سینوس ماگزیلاری و میزان پنوماتیزاسیون استخوان آلوئول در ارتباط با سن و جنس بود. مواد و روشها: در این مطالعه، تصاویر CT اسکن سینوسهای ماگزیلاری 199 بیمار بالغ بررسی شد. در تصاویر کرونالCT اسکن، میزان پنوماتیزاسیون استخوان آلوئول در محل هر دندان خلفی اندازهگیری شد و حجم سینوسهای ماگزیلاری به صورت سه بعدی با نرم افزار Amira محاسبه گردید. دادهها با آزمون همبستگی Spearman و تحلیل رگرسیون مورد تحلیل آماری قرار گرفت. یافته ها:در این مطالعه از 199 بیمار مورد بررسی، 159 نفر آقا در بازه سنی 92-18 سال و 40 نفر خانم در بازه سنی 68-18 سال بودند. اندازهگیریها نشان داد که متوسط حجم سینوس ماگزیلاری در آقایان80/5 ±32/15 میلیلیتر و در خانمها 91/4 ±02/13 میلیلیتر بود و این اختلاف از لحاظ آماری در سمت راست و چپ معنی دار بود. (001/0P-value = و 008/0 P-value = به ترتیب). همبستگی معناداری بین سن و میانگین حجم سینوس ماگزیلاری بدست آمد (03/0=P-value و 15/0- = r) که بیانگر این بود که با افزایش سن میزان حجم سینوس کاهش مییابد. ضریب همبستگی بالایی میان حجم سینوس ماگزیلاری و مقدار پنوماتیزاسیون استخوان آلوئول بدست آمد. (58/0 = r راست و 72/0 = r چپ). علیرغم بالاتر بودن میانگین پنوماتیزاسیون در آقایان، این اختلاف از نظر آماری معنیدار نبود، همچنین بین سن و میانگین پنوماتیزاسیون در سمت راست و چپ ضریب همبستگی معنیداری بدست نیامد. ( 05/0 < P-value) نتیجه گیری: یافتههای این مطالعه نشان دادند که حجم سینوسهای ماگزیلاری در آقایان بیشتر از خانمها بوده و این حجم با افزایش سن کاهش مییابد. میانگین پنوماتیزاسیون استخوان آلوئول ارتباطی با سن ندارد و بین دو جنس دارای اختلاف قابل توجهی نمی باشد.

بررسی اثر لیزر Er: YAG و یون فلوراید بر رمینرالیزاسیون ضایعات پوسیدگی اولیه مینایی
مقدمه:این مطالعه آزمایشگاهی اثر لیزر Er:YAG و ژل فلوراید اسیدی را روی رمینرالیزاسیون ضایعات پوسیدگی اولیه مینایی، هنگامی که به تنهایی و یا با هم بکار می روند، بررسی کرده است. مواد و روشها: 90 اسلب باکال و لینگوالی از دندانهای مولر انسانی تهیه گردید. نمونه ها تحت سیکل pH قرار گرفتند تا ضایعات پوسیدگی اولیه مینایی ایجاد گردد. سپس به صورت تصادفی به شش گروه به صورت زیر تقسیم شدند: گروه اول: کنترل مثبت بدون ضایعات پوسیدگی اولیه، گروه دوم: کنترل منفی دارای ضایعات پوسیدگی اولیه مینایی، گروه سوم: قرار گرفتن ژل فلوراید روی مینا، گروه چهارم: تاباندن لیزر Er:YAG روی مینا، گروه پنجم: ابتدا استفاده از ژل فلوراید و سپس تاباندن لیزر، گروه ششم: ابتدا لیزر و سپس کاربرد ژل فلوراید. قبل و بعد از درمان، محتوای یون فلوراید نمونه ها به وسیله دستگاه پتانسیومتر اندازه گیری شد. آنالیز آماری با آزمونهای کروسکال والیس وANOVA انجام گرفت. (05/0= ) یافته ها: کاربرد ژل فلوراید قبل یا بعد از تاباندن لیزر بیشترین افزایش را در مقاومت به اسید و افزایش جذب فلوراید مینایی، مشخص کرد. (05/0 (p<. گروه چهارم با تاباندن لیزر، افزایش جذب فلوراید مینایی را در مقایسه با گروه اول و دوم نشان داد. اما گروه درمان شده با لیزر در مقایسه با گروه درمان شده با ژل فلوراید اختلاف مشخصی نداشتند. نتیجه گیری:لیزر Er:YAG ترکیب شیمیایی مینا را تغییر داده و سختی آن را، به خصوص در ترکیب با فلوراید افزایش میدهد که پیشنهاد کننده پتانسیل ضدپوسیدگی آن است.

بررسی اثر Bevel و زوایای لبه ای حفره در توزیع استرس ترمیمهای کامپوزیتی غیرمستقیم به روش آنالیز اجزای محدود
مقدمه:از روش آنالیز اجزای محدود میتوان برای نحوه توزیع استرس در دندان استفاده نمود. این مطالعه برای بررسی توزیع استرس در ترمیمهای غیرمستقیم رزینی حفرههای مختلف کلاس II برای بدست آوردن زاویه بهینه تراش دیواره و همچنین بررسی تاثیر بوول زاویه آگزیوپالپال با استفاده از آنالیز اجزای محدود سه بعدی انجام شد. مواد و روشها: یک مدل سه بعدی مولر اول مندیبل سالم و سه مدل حفره کلاس II مزیواکلوزودیستال با زاویه تراش لبه ای مختلف ایجاد شدند. همچنین همه مدلهای تراش با بوول در زاویه اگزیوپالپال بررسی شدند. استرسهای مختلف بر دندان مولر اول مندیبل که با زوایای تراش دیواره مختلف در محیط نرمافزار شبیه سازی شده بود، وارد گردید. نیروی اکلوزالی عمودی100 نیوتن اعمال شد. خصوصیات تمام مواد ایزوتروپیک و الاستیک تعریف شد و با کمک یک تحلیل خطی استاتیک توزیع تنش و حداکثر استرس ایجاد شده در هر مدل بر حسب مگا پاسکال بدست آمد. دادهها با استفاده از شاخصهای توصیفی ارائه شد. یافته ها:حداکثر استرس در دندان سالم شبیه سازی شده برابر 63/24 مگا پاسکال بود. توزیع تنش مشخصکننده تجمع فشار در مرکز و ناحیه فورکای دندان مولر اول مندیبل و همچنین در کاسپها در ناحیه اکلوزال بود. در مدل با بوول زاویه آگزیو پالپال با زاویه لبهای تراش 95 درجه، حداکثر استرس 90/24 مگا پاسکال بود که کاهش 05/0 استرس نسبت به مدل بدون بوول آگزیوپالپال را نشان داد. همچنین در زاویه تراش لبهای 5/97 درجه، حداکثر استرس 85/24 مگاپاسکال در مدل بدون بوول به حداکثر استرس 80/24 مگاپاسکال در مدل با بوول رسید که سبب کاهش 05/0 استرس گردید. در زاویه لبه ای 100 درجه، از حداکثر استرس 30/25 مگاپاسکال در مدل بدون بوول به حداکثر استرس 20/25 مگاپاسکال در مدل با بوول آگزیوپالپال رسید که باعث کاهش 1/0 مگاپاسکال استرس شد. نتیجه گیری: با توجه به محدودیتهای این قبیل مطالعات نرم افزاری، بهترین نتیجه درمانی بدست آمده در تراش دیوارهها با زاویه خطی 5/97 درجه می باشد. همچنین بوول آگزیوپالپال باعث کاهش حداکثر استرس در ترمیم شده و احتمال شکست آن را کاهش می دهد.

بررسی بیان TGF-β در ژانت سل گرانولومای فکین و ژانت سل تومور استخوان
مقدمه: ژانت سل گرانولومای محیطی(PGCG)، ژانت سل گرانولومای مرکزی (CGCG) فکین و ژانت سل تومور (GCT) استخوان ضایعاتی با نمای هیستوپاتولوژی مشابه ولی رفتار بیولوژیک متفاوت میباشند.هدف از مطالعه حاضر بررسی میزان بیانTGF-β در این ضایعات به روش ایمنوهیستوشیمی بود. مواد و روشها: در مطالعه حاضر، 15 نمونه PGCG، 15 نمونه CGCG و 10 نمونه GCT بررسی شد. ایمنوراکتیویتی TGF-β در سلولهای تک هستهای (MC) و چند هستهای (MGC) گروههای مورد مطالعه ارزیابی گردید. آزمونهای Kruskal-Wallis و تست تعقیبی Dunn، برای مقایسه میزان بیان TGF-β در گروههای مورد مطالعه استفاده شد. یافته ها:بین سه گروه مورد مطالعه تفاوت آماری معناداری هم در مورد سلولهای چندهستهای و هم تکهستهای مشاهده شد. بیشترین میانگین رنگپذیری TGF-β مربوط به GCT و کمترین مربوط بهPGCG بود. در مقایسه دو به دو گروهها، بین گروه PGCG و GCT هم برای سلولهای چندهسته ای و هم تک هسته ای ارتباط معنادار مشاهده گردید، در حالیکه بین گروههای CGCG و GCT فقط در مورد سلولهای تک هسته ای ارتباط معنادار گزارش شد. نتیجه گیری: یافته های این مطالعه میتواند نشان دهنده رفتار بیولوژیک و پاتوژنز متفاوت ضایعات مورد مطالعه باشد.

بررسی آزمایشگاهی تأثیر روشهای مختلف نوردهی بر استحکام باند برشی ونیر کامپوزیتی به مینا و عاج
مقدمه:ونیر کامپوزیتی به عنوان یک گزینه درمانی زیبا و محافظهکارانه برای اصلاح دندانهای تغییر رنگ یافته و بد شکل مورد استفاده قرار میگیرد. با توجه به تأثیر روش نوردهی بر میزان استرس ناشی از انقباض پلیمریزاسیون، مطالعه فوق با هدف تعیین تأثیر روشهای مختلف نوردهی بر استحکام باند برشی کامپوزیت به مینا و عاج طرح ریزی شد. مواد و روشها: در این مطالعه آزمایشگاهی، 72 دندان پرهمولر به دو گروه مساوی مینایی و عاجی تقسیم شدند. در هر کدام از گروهها، سطوح باکال تا حصول یک سطح صاف مینایی یا عاجی ساییده شده و سپس نمونهها در مکعبهای آکریلی مانت شدند. سطوح به باندینگ Single Bond 2 آغشته و کیور گردیدند. بر اساس سه روش نوردهی Conventional، Soft-start و Pulse-delay، نمونههای هر دو گروه به سه زیرگروه تقسیم شدند (12n=). پس از فرمدهی یک استوانه از کامپوزیت Z250 بر روی سطوح با استفاده از روشهای نوردهی متفاوت، استحکام برشی نمونهها اندازهگیری شد. دادهها با آنالیز واریانس یک طرفه به همراه آزمون مقایسات چندگانه Tukey تحلیل شدند (05/0=α) یافته ها:آزمون واریانس یک طرفه تفاوت آماری معنیداری در استحکام باند برشی کامپوزیت به مینا بین زیرگروههای مینایی نشان نداد (185/0P=)،حال آنکه تفاوت استحکام باند برشی کامپوزیت به عاج بین زیرگروههای عاجی معنیدار بود (042/0P=). بیشترین نوع شکست در بین تمامی زیرگروهها مربوط به نوع ادهزیو بود. نتیجه گیری: بر اساس نتایج این مطالعه، نوردهی به روش Pulse-delay و Soft-start در هر دو گروه مینایی و عاجی میتواند باعث افزایش استحکام باند برشی گردد. این امر در گروههای عاجی مصداق بیشتری دارد.

بررسی تأثیر کشیدن دندانهای پرمولر اول در بیماران تحت درمان ارتدنسی ثابت بر روی شاخصهای مورفومتریک لبخند
مقدمه:ما در این تحقیق بر آن شدیم تا تغییرات شاخصهای مورفومتریک لبخند، قبل و بعد از ارتدنسی ثابت با و بدون کشیدن دندان را بررسی کنیم. مواد و روشها: این مطالعه گذشته نگر، برروی فتوگرافهای قبل و بعد از درمان ارتدنسی46 بیمار خانم 18 تا 25 ساله انجام شد.26 نفر از این بیماران با کشیدن دندانهای پرمولر (Ext) و 20 نفر دیگر بدون کشیدن دندان (Nonext) درمان شده بودند و درمان ارتدنسی در تمام بیماران به کمک سیستم رات صورت گرفته بود. فتوگرافهای لبخند اجتماعی مربوط به قبل و بعد از درمان این بیماران توسط نرمافزار smile analyzer مورد ارزیابی قرار گرفت و شاخصهای مورفومتریک لبخند در آنها اندازهگیری شد. سپس جهت مقایسه این شاخصها قبل و بعد از درمان در هر گروه از paired t test و به منظور مقایسه دو گروه با یا بدون کشیدن ازindependent T test استفاده گردید. یافته ها: نتایج این تحقیق نشان داد که تغییرات شاخصهای مورفومتریک لبخند قبل و بعد از درمان در هریک از دو گروه با یا بدون کشیدن معنی دار نبود (05/0(P=. با مقایسه دو گروه با یا بدون کشیدن که با نرم افزار smile analyzer انجام شد مشخص گردید که در مقادیر قبل از درمان تنها عرض دنتیشن قابل رویت فک بالا (01/0(P= و در مقادیر بعد از درمان عرض دنتیشن قابل رویت فک بالا (001/0 (P=، عرض لبخند (04/0 (P=و ارتفاع لبخند (04/0 (P= اختلاف آماری معنیداری داشتند و هر چهار معیار در بدون کشیدن دندان نسبت به گروه دارای کشیدن بیشتر بود. معیارهای دیگر در دو گروه با یکدیگر اختلاف آماری معنیداری نداشتند. نتیجه گیری: درمان ارتدنسی با سیستم رات در دو گروه با یا بدون کشیدن، باعث تغییرات معناداری در شاخصهای مورفومتریک لبخند نمیگردد.

تظاهر زنجیره لامینین 5 گاما 2 درکارسینوم سلول سنگفرشی دهان و وروکوس کارسینوما و دیسپلازی مخاط دهان
مقدمه:کارسینوم سلول سنگفرشی حفره دهان(OSCC) از شایعترین سرطانهای حفره دهان در سراسر جهان میباشد. اصلیترین اجزای تشکیلدهنده تیغه پایه، شامل کلاژن نوع 4 و گلیکوپروتئین چسبنده لامینین میباشند. از دست رفتن موضعی ساختار غشاء پایه، باعث بروز لامینین 5 گاما 2 از طریق پروتئولیز غشاء پایه و یا سنتز در سیتوپلاسم سلولهای نئوپلاستیک میشود. لامینین 5 گاما 2 پروتئینی است که نقش مهمی در جابجایی سلولهای نئوپلاستیک در طول تهاجم تومور ایفا میکند و بیان آن در سلولها و استرومای مجاور تومورال میتواند تهاجم سلولی را تأیید کند. هدف از مطالعه حاضر، ارزیابی تظاهر لامینین 5 گاما 2 در کارسینوم سلول سنگفرشی دهان(OSCC) و وروکوس کارسینوما و مخاط دیسپلاستیک در مقایسه با بافت سالم بود. مواد و روشها:تعداد 57 بلوک پارافینه سلول سنگفرشی، 31 نمونه OSCC، 9 مورد وروکوس کارسینوما، 9 مورد دیسپلازی و 8 مورد مخاط نرمال مجاور بافت تومورال (به عنوان گروه کنترل)، جهت نشانگر لامینین 5 گاما 2، اسکوربندی شد. آزمونهای آماری کای اسکوئیر و من-ویتنی مورد استفاده قرار گرفت و توسط نرم افزار SPSSنسخه 20 تجزیه وتحلیل شد. یافته ها:میزان تظاهر نشانگر لامینین در اپیتلیوم نمونههای SCC در 3/61 % ، در نمونههای وروکوس کارسینوما در 3/33 % و در دیسپلازی در 2/22 % مثبت و در مخاط نرمال100 % منفی بود. توزیع فراوانی منفی و مثبت بودن لامینین برحسب نوع ضایعه بررسی شد. سطح معناداری برابر با 006/0 محاسبه شد که کمتر از 05/0 بود. در ضایعههای مختلف تعداد مثبتهای لامینین تفاوت معنادار داشتند و بیشترین تعداد مثبت مربوط به ضایعه SCC بود. نتیجه گیری: تظاهر لامینین 5 گاما 2 در غشاء پایه، ماتریکس خارج سلولی و اپیتلیوم بافت سرطان بدخیم، خبر از رفتار تهاجمی و متاستاز سرطان در OSCC می دهد.

بررسی مقاومت به شکست کامپوزیت رزینهای bulk-fill با ادهزیوهای توتال اچ و سلف اچ در ترمیمهای کلاس دو
مقدمه:مقاومت به شکست مناسب، فاکتور عمدهای در موفقیت طولانی مدت درمانهای ترمیمی میباشد. هدف این مطالعه، بررسی میزان مقاومت به شکست حفرات کلاس دو، ترمیم شده با ادهزیو توتال اچ و سلف اچ با انواع کامپوزیتهایBulk-fill بود. مواد و روشها: در این مطالعه آزمایشگاهی، بر روی 40 دندان پره مولر سالم انسانی با اندازه های تقریباً یکسان حفرات کلاس دو کانونشنال بر روی یکی از سطوح پروگزیمالی دندانها تراشیده شدند. نیمی از حفرات با کامپوزیت رزین ویسکوز (Sonic Fill nanohybrid) Bulk-fill با ادهزیوهای توتالاچ(Optibond Solo Plus Kerr) یا سلفاچ (Optibond XTR, Kerr)و نیمی دیگر با کامپوزیت رزین Tetric N-Ceram (Ivoclar vivadent) با ادهزیوهای توتال اچ یا سلف اچ به روش توده ای ترمیم شدند. برای آزمون مقاومت به شکست، نمونه ها در دستگاه اینسترون قرار گرفتند. مقادیر نیروی ثبت شده در هنگام شکست جهت مقایسه گروههای آزمایشی ثبت شدند. داده های بدست آمده با آنالیز واریانس دو عاملی مورد واکاوی آماری قرار گرفتند. یافته ها: به ترتیب بیشترین و کمترین مقادیر مقاومت به شکست مربوط به کامپازیت Tetric N-Ceram با ادهزیوهای توتال اچ و سلف اچ بود. تفاوت معناداری در مقاومت به شکست کامپوزیتهای Tetric N-Ceram و Sonic fill با ادهزیوهای توتال اچ و سلف اچ وجود نداشت. (05/0(P>. نتیجه گیری: هر دو نوع کامپوزیت Bulk-fill با ادهزیوهای مختلف، مقادیر مقاومت به شکست مشابهی داشتند.

مقایسه آزمایشگاهی اثر ضد قارچی عصاره گیاه پونه کوهی با نیستاتین بر کاندیدا آلبیکنس
مقدمه:عفونتهای دهانی ایجاد شده توسط گونههای کاندیدا به طور گسترده ای در حال افزایش است. یکی از درمانهای موضعی رایج ضایعات دهانی کاندیدیازیس، نیستاتین است که دارای فعالیت ضدقارچی و ضدمیکروبی می باشد. درمانهای ضدقارچی معمول دارای اثرات جانبی متعددی بوده و مقاومت دارویی و عود مکرر عفونت کاندیدیایی را موجب میشوند. بنابراین استفاده از راهکارهای مناسب برای جایگزینی داروهای سنتتیک که این مشکلات را از بین ببرد، مورد توجه بوده است. هدف این مطالعه، مقایسه آزمایشگاهی اثر ضدقارچی عصاره گیاه پونه کوهی با نیستاتین بر کاندیدا آلبیکنس بود. مواد و روشها: در این مطالعه آزمایشگاهی، عصاره آبی و الکلی گیاه پونه کوهی در غلظتهای 10، 20، 40، 80 و 100 درصد تهیه گردید. کاندیدا آلبیکانس در محیط کشت دکستروز آگار حاوی کلرامفنیکل کشت داده شد و داخل هر پلیت کشت، یک عدد دیسک حاوی هر یک از غلظتهای مورد استفاده از عصاره گیاه، یک عدد دیسک نیستاتین 100 واحدی به عنوان کنترل مثبت و یک عدد دیسک حاوی آب مقطر به عنوان کنترل منفی قرار داده شد. هر یک از غلظتهای مورد بررسی، در 10 پلیت کشت، به طور جداگانه تکرار شدند. بعد از 24 ساعت، میانگین قطر هاله عدم رشد غلظتهای مختلف عصاره و نیستاتین با استفاده از آنالیز واریانس یک طرفه و یا آزمون t دو نمونه مستقل مقایسه شد. یافته ها:میانگین قطر هاله عدم رشد پیرامون دیسکهای حاوی عصاره های آبی و الکلی پونه کوهی در تمامی غلظتهای مورد استفاده، به طور معنیداری کمتر از نیستاتین بود (001/0>P). نتیجه گیری:اثر ضدقارچی داروی نیستاتین بر روی کاندیدا آلبیکنس، در مقایسه با عصاره آبی و الکلی پونه کوهی بیشتر بود. کلمات کلیدی: کاندیدا آلبیکانس، نیستاتین، پونه کوهی.
