تعیین میزان دستیابی به اهداف آموزشی گروههای پریو و ترمیمی دانشکده دندانپزشکی زاهدان در سال 94 – 93
مقدمه: ارزشیابی یک فرآیند مستمر می باشد که درک، ارتقای یادگیری وتغییرات لازم برای دستیابی به اهداف آموزشی را مورد توجه قرار می دهد. هدف مطالعه حاضر بررسی توانمندی دانشجویان دندانپزشکی دانشگاه علوم پزشکی زاهدان از طریق تعیین نظرات آنها نسبت به میزان دستیابی به اهداف یادگیری دروس نظری وعملی پریودانتیکس وترمیمی در سال تحصیلی 94-93 بود . مواد و روش ها: در این مطالعه مقطعی، توصیفی- تحلیلی که در سال تحصیلی 94 -93 انجام شد، 100 نفر دانشجوی دندانپزشکی که همه واحدهای تئوری و عملی آموزشی پریودونتولوژی و ترمیمی را گذرانده بودند، به عنوان جمعیت مورد مطالعه، درنظر گرفته شدند. سپس پرسشنامه هایی در هر یک از دروس پریودونتولوژی و ترمیمی طراحی گردید. پرسشنامه پریودونتولوژی با روایی و پایایی 9/0 (9/0=CVR و 9/0=CVI) و پرسشنامه گروه ترمیمی با روایی و پایایی 8/0 (8/0=CVR و 8/0=CVI) مورد تایید قرار گرفتند (سوالات پرسشنامه به صورت جداول در انتها ضمیمه شده است). یافته ها: میانگین نمرات توانمندی دانشجویان از دیدگاه خودشان برای دستیابی به اهداف درس پریودنتولوژی، 5 درصد عالی، 30 درصد خوب، 56 درصد متوسط و 9 درصد ضعیف بود و برای درس ترمیمی 17 درصد عالی، 39 درصد خوب، 34 درصد متوسط و 10 درصد ضعیف بود. نتیجه گیری: با توجه به نتایج بالا، از دیدگاه دانشجویان، توانمندی آنان در دستیابی به اهداف آموزشی مطلوب نبود. با مشخص شدن نقاط ضعف و قوت دانشجویان در هریک از دروس پریودنتولوژی و ترمیمی، به نظر میرسد برای برنامه آموزشی این بخش ها نیاز به بازنگری و رفع نقایص باشد.

بررسی تأثیر اضافه کردن لیزر کم توان به درمان دارویی در بیماران مبتلا به سندرم سوزش دهان: کارآزمایی بالینی تصادفی
مقدمه: هدف از مطالعه حاضر بررسی تأثیر اضافه کردن تابش لیزر کم توان به درمان دارویی در بیماران مبتلا به سندرم سوزش دهان (BMS) بود. مواد و روش ها: در این کارآزمایی بالینی تصادفی یک سویه کور، تعداد 20 بیمار (17 زن و 3 مرد) مبتلا به BMS انتخاب و به طور تصادفی به دو گروه تقسیم شدند. در تمام بیماران، درمان دارویی با سیتالوپرام تجویز شد. در گروه تجربی، تابش با لیزر کم توان InGaAlP (nm660، mW 200، J/cm2 3) به مدت 4 هفته (12 جلسه) انجام شد. در گروه کنترل، درمان مشابه اما توسط پروب لیزر خاموش انجام گردید. در پایان هر هفته و نیز 1، 2 و 3 ماه بعد از لیزر درمانی، شدت و مدت سوزش دهان از بیمار پرسیده شد. داده ها با استفاده از آزمونهای t-test ، Paired t-test و همبستگی پیرسون در نرم افزار SPSS آنالیز شد. یافته ها:شدت و مدت زمان سوزش در شبانه روز در هیچ یک از فواصل درمان و پیگیری تفاوت معنی داری بین دو گروه نداشت (05/0<P). در پایان دورۀ پیگیری نسبت به شروع مطالعه، در هر یک از گروه ها کاهش معنی داری در شدت و مدت زمان سوزش مشاهده شد (001/0>P). درصد بهبودی افراد در گروه تجربی در پایان دورۀ پیگیری 81 درصد و در گروه کنترل 70 درصد محاسبه شد و تفاوت معنیداری بین دو گروه نداشت (05/0<P). بین میزان بهبودی و میزان سوزش اولیه رابطۀ معنی داری مشاهده شد (001/0>P). نتیجه گیری: تحت شرایط این مطالعه، داروی سیتالوپرام در کاهش شدت و مدت زمان سوزش دهان در بیماران مبتلا به BMS موثر بود. اضافه کردن تابش لیزر کم توان به روند درمان با سیتالوپرام، تأثیری فراتر از لیزر پلاسبو در کاهش علایم بیماران BMS نداشت.

بررسی آزمایشگاهی استحکام شکست پروتزهای پارسیل ثابت متکی بر ایمپلنت با ابعاد کانکتورهای مختلف در سیستم zir/CAD/CAM
مقدمه: تاثیر ابعاد کانکتورهای مختلف در سیستم های تمام سرامیکی مورد بحث است. هدف از انجام این بررسی، ارزیابی تاثیر سایز های مختلف کانکتور در مقاومت به شکست بریج های خلفی اکسیدزیرکونیوم بود. مواد و روش ها: در این مطالعه تجربی – آزمایشگاهی، 24 عدد بریج خلفی زیرکونیا و 8 عدد بریج خلفی متال سرامیک با ساپورت ایمپلنت (گروه کنترل) با تکنولوژی CAD/CAM جهت جایگزینی پره مولر دوم ساخته شدند. نمونه ها به چهار گروه با اندازه کانکتور متفاوت تقسیم شدند. نیروی شکست نمونه ها، پس از 1 روز نگهداری در بزاق مصنوعی و اعمال سیکل های حرارتی، اندازه گیری شد. تست های Tukey HSD وANOVA جهت آنالیز داده ها استفاده شدند. یافته ها: مقادیر میانگین مقاومت به شکست گروه متال سرامیک به طور معنی داری بیشتر ازگروه های تمام سرامیک بود. در گروه های Zir/CAD/CAM مقادیر میانگین مقاومت به شکست گروه با اندازه کانکتور mm 4×4 به صورت معنی داری بیشتر از گروه های با اندازه کانکتور mm3×3 و mm3×2 بود. نتیجه گیری: مقادیر میانگین نیروی شکست نمونه های زیرکونیایی بصورت معنی داری تحت تاثیر اندازه کانکتور (001/0>P) قرار داشت.

درمان بیماران مبتلا به حساسیت عاجی افزایش یافته با روشی جدید و مقایسه آن با لیزر: مطالعه کلینیکی
مقدمه: افزایش حساسیت عاجی یک حساسیت با درد کوتاه و تیز در دندان هاست که با اعمال تحریکاتی مانند استفاده از غذاها و نوشیدنیهای سرد، می تواند ایجاد شود. روشهای مختلفی برای درمان حساسیت عاجی پیشنهاد شده است ولی هنوز توافق قطعی در این زمینه وجود ندارد. هدف از این مطالعه مقایسه کلینیکی دو روش درمانی با استفاده از لیزر Nd:YAG و ژل پروپولیس 40 درصد در بهبود علائم افزایش حساسیت عاجی است. مواد و روشها: 25 بیمار دارای افزایش حساسیت عاجی با استفاده از روش نمونه گیری آسان انتخاب شدند. به روش Split mouth دندانهای حساس یک سمت با لیزر Nd:YAG و سمت دیگر با استفاده از ژل پروپولیس درمان شد. هر بیمار سه جلسه درمان با فواصل یک هفته دریافت کرد و بعد از یک ماه فالوآپ شد. برای سنجش میزان درد بیماران قبل، حین و بعد از درمان، از مقیاس VAS استفاده شد. داده ها با آزمونهای t زوجی و تحلیل اندازه های مکرر آنالیز شدند. یافتهها: کاهش حساسیت پس از درمان در هر دو گروه معنیدار بود. پس از یک ماه حساسیت در سمت درمان با لیزر از 48/4 به 56/0 و در سمت پروپولیس از 40/4 به 48/0 کاهش یافت. ولی بین گروه درمانی لیزر و پروپولیس تفاوت معنیداری وجود نداشت ( 05/0<P). نتیجه گیری: دو درمان انجام شده، در درمان حساسیت عاجی تفاوت چشمگیر نداشتند. بنابراین میتوان ژل پروپولیس را به عنوان درمانی ساده و کم هزینه به جای درمان هزینه بر و با تکنیک حساستر لیزر معرفی کرد.

تقلب علمی در انجام پروژه دانشگاهی؛ مطالعه ای در میان دانشجویان سال ششم دانشکده دندانپزشکی مشهد
مقدمه: سوءرفتار دانشگاهی در میان دانشجویان مشکلی جدی در دانشگاهها و مراکز آموزش عالی محسوب میشود. هدف این مطالعه توصیف شیوههای تقلب و میزان شیوع این پدیده، تحلیل علل وقوع، و نحوه مواجهه با آن از زبان دانشجویان دندانپزشکی سال ششم است.
مواد و روش ها: در این مطالعه پرسشنامهای باز استفاده شده است که در آن سابقه یک پروژه آموزشی مورد تشکیک مربوط به همین گروه از دانشجویان توصیف شد و بر اساس آن پنج پرسش پیش روی دانشجویان قرار گرفت. علاوه بر تحلیل فراوانی پاسخها، متن پاسخ دانشجویان به صورت کیفی تحلیل محتوا شد.
یافتهها: از میان 77 دانشجوی شرکتکننده در مطالعه، 9/77 درصد از دانشجویان صراحتا وقوع تقلب دانشجویان در پروژه سال گذشته را پذیرفته بودند. دانشجویان سه دسته نکات مرتبط با دانشجو، مرتبط با استاد و موضوعات کلان را به عنوان علل وقوع این پدیده برشمردند. اکثریت قاطع دانشجویان (7/98 درصد) رفتار احتمالی دانشجویی را که وقوع تقلب همکلاسی هایش را به استاد گزارش دهد غیرقابل قبول دانستند، اما دانشجویان درباره میزان حساسیتی که استاد بایستی در مقابل سوءرفتارهای دانشگاهی از خود نشان دهد اتفاق نظر نداشتند. همچنین راهکارهای پیشنهادی دانشجویان ذیل سه دسته "پیشنهادهای تکنیکی برای ارائه جلسات"، "پیشنهادها در مورد نحوه مواجهه استاد با موقعیت های مشابه" و " پیشنهادهای کلی" طبقهبندی شد.
نتیجهگیری: اساتید و مسوولین آموزشی دانشگاه با توجه به نکات دانشجویان میتوانند در پیشگیری از وقوع سوءرفتارهای دانشگاهی و مواجهه صحیح با این قبیل پدیدهها راهکارهای اثربخش جدیدی را اتخاذ کنند.

مقدمه: اختلاف زیاد خواص مکانیکی کراون با عاج دندان در ترمیم های پست-کور کراون منجر به تمرکز تنش در عاج و مارجین سرویکال می شود. به منظور کاهش تنش در عاج و مارجین سرویکال، نوعی از مواد غیرهمگن (مواد هدفمند) برای کراون پیشن هاد شده است. هدف مطالعه حاضر، تعیین تاثیر کراون های غیرهمگن روی توزیع تنش ها در ریشه و مارجین سرویکال به کمک آنالیز اجزای محدود بود. مواد و روش ها: یک مدل کامپیوتری از دندان پرمولر اول پایین و بافت های اطراف آن شامل لیگامان پریودنتال، استخوان های کورتیکال و ترابیکولار تهیه شد. کراون های همگن و غیرهمگن به کمک آنالیز اجزاء محدود بررسی و توزیع تنش های آن ها با یکدیگر مقایسه شد. یافته ها: کراون آلومینا منجر به بیشترین تمرکز تنش در عاج و مارجین سرویکال شد؛ درحالی که کراون غیرهمگن درجهبندی شده با 10P= (10FGM) منجر به کمترین تمرکز تنش در عاج و مارجین سرویکال شد. نتیجه گیری:نتایج آنالیز اجزاء محدود نشان داد که کراون های غیرهمگن نسبت به کراون های همگن برتری دارند. کراون های غیرهمگنی که به درستی طراحی شده باشند می توانند منجر به کاهش تنش ها در عاج و مارجین سرویکال و در نتیجه کاهش احتمال شکست ریشه یا ترمیم شوند.

ارزیابی سطح سرمی سلنیوم، منیزیوم، سدیم، روی و مس در بیماران با اختلالات مفصل گیجگاهی فکی
مقدمه: محتوای سرمی ریزمغذی ها در طی مسیر التهاب و عفونت تغییر می کند. در این مطالعه ارزیابی سطح سرمی سلنیوم، منیزیوم، سدیم، روی و مس در بیماران دارای اختلال مفصل گیجگاهی فکی و مقایسه آن با افراد سالم انجام گرفت. مواد و روش ها: در این مطالعه مورد- شاهدی، از بین بیماران دارای اختلال مفصل گیجگاهی فکی مراجعه کننده به دانشکده دندانپزشکی مشهد، تعداد 60 نفر انتخاب شدند. همه افراد شرکت کننده توسط یک متخصص پروتز با تجربه بالا در زمینه اکلوژن معاینه شدند. 20 نفر دارای اختلال عضلانی، 20 نفر دارای اختلال مفصلی (دررفتگی دیسک قابل برگشت) و 20 نفر دارای اختلال التهابی (استئوآرتریت) بودند، همچنین 20 نفر از افراد سالم به عنوان گروه شاهد انتخاب شدند. در همه این افراد سطوح سرمی سلنیوم، منیزیوم، سدیم، روی و مس به کمک Flame atomic absorption spectrometry (FAAS) بررسی گردید. از آزمون کروسکال والیس، کای اسکوئر و دقیق فیشر جهت آنالیز آماری داده ها استفاده شد. (05/0˂P) یافته ها: میانگین سنی افراد شرکت کننده در مطالعه 31/9±7/31 سال بود. 25 نفر (6/31 درصد) مرد و 54 نفر (4/68 درصد) زن شرکت کردند. میانگین سطح سرمی عناصر روی، منیزیوم، سدیم، مس و سلنیوم در افراد دارای اختلال عضلانی به ترتیب 68/67، 94/1، 05/140، 74/122 و 08/112، در افراد دارای اختلال جابجایی دیسک به ترتیب 65، 94/1، 95/138، 94/116 و 79/110، در افراد دارای اختلال التهابی استئوآرتریت به ترتیب 65/65، 84/1، 50/136، 5/138 و 75/105 و در افراد گروه کنترل به ترتیب 4/62، 8/1، 4/135، 51/138 و 58/110 میکروگرم بر دسی لیتر بود. نتیجه گیری: میانگین سطح سرمی عنصر سدیم در گروه عضلانی نسبت به گروه های استئوآرتریت و کنترل و همچنین در گروه اختلال مفصلی (جابجایی دیسک قابل برگشت) نسبت به گروه کنترل به طور معنی داری بالاتر بود.

استفاده از خدمات دندانپزشکی و هزینه های پرداخت از جیب در مراجعین به کلینیک های دندان پزشکی رامسر
مقدمه: مراقبت های دندانپزشکی در تمام دنیا گران و پرهزینه محسوب می شود که بخش عمده ای از آن بر عهده بیماران است. این مطالعه با هدف تعیین میزان استفاده و هزینه های پرداخت از جیب برای خدمات دندان پزشکی در کلینیک های دندان پزشکی شهر رامسر در سال 1394 انجام شد. مواد و روشها: در این مطالعه مقطعی، 302 بیمار مراجعه کننده به کلینیک های دندان پزشکی شهر رامسر به روش نمونه گیری طبقه ای انتخاب شدند. اطلاعات مورد نیاز از طریق بررسی پرونده بیمار و مصاحبه تلفنی گردآوری شد. تجزیه و تحلیل داده ها با استفاده از روش های آمار توصیفی و آزمون رگرسیون در نرم افزارSPSS با ویرایش 21انجام شد. یافتهها: بیشترین میزان استفاده از خدمات دندانپزشکی، با 9/66 درصد مربوط به خدمات ترمیمی و کمترین درمان مربوط به ارتودنسی بود. خدمات معالجه ریشه، ارتودنسی و پروتز بیشترین سهم پرداخت از جیب را داشتند و بیشترین پوشش بیمه ای با ۳۰ درصد مربوط به خدمات جراحی بود. بطور کلی سهم پرداخت از جیب برای هزینه خدمات دندانپزشکی، ۹۱ درصد بود و وضعیت اشتغال، سطح تحصیلات، درآمد سالانه خانوار، و وجود فرد بالای 65 سال در خانوار تاثیر معنی داری برای میزان پرداخت از جیب داشت(05/0≥P). نتیجه گیری: با توجه به بالا بودن پرداخت از جیب بیماران، نیاز به اتخاذ سیاست های جدید در پوشش بیمه ای خدمات دندانپزشکی می باشد. همچنین سیاست های ملی متمرکز بر خدمات دندانپزشکی پیشگیری، طراحی و راه اندازی کلینیک های دولتی و اولویت دادن به گروه های کم درآمد در برنامه های بیمه ای پیشنهاد می شود.

رابطه بین اکلوژن تروماتیک و شکست در ترمیم ضایعات پری اپیکال: یک گزارش مورد
مقدمه: موفقیت دندان های درمان ریشه شده با ضایعات پری اپیکال در مطالعات بسیاری گزارش شده است. هر چند در برخی موارد مانند اکلوژن تروماتیک، ضایعات بدون تغییر باقی بمانند. به عبارت دیگر اثر اکلوژن تروماتیک در پیشرفت بیماری های پالپ و پری اپیکال به طور کامل شناخته نشده است. گزارش مورد: این مقاله باقی ماندن ضایعه پری اپیکال و علائم بیمار را بعد از درمان اندو و جراحی مناسب در یک بیمار 17 ساله گزارش می کند که بعد از حذف تماس های پیش رس، علائم بهبود پیدا کرد و ضایعه پری اپیکال ترمیم شد. نتیجه گیری: رفع تماس های پیش رس در طی مراحل درمان می تواند یکی از فاکتورهای تأثیرگذار بر نتایج درمان باشد.

درمان ترکیبی یک مورد نادر فیبروز تحت مخاطی کودکان: گزارش مورد
مقدمه: فیبروز تحت مخاطی دهان (Oral submucos fibrosis (SMF)) یک عارضه غیر قابل برگشت، مزمن، پیش سرطانی و پیشرونده است. تظاهرات بالینی آن به صورت اسکار و فیبروز پیشرونده مخاط حفره دهان می باشد. همچنین SMF می تواند منجر به بروز عوارضی مانند احساس سوزش در دهان، عدم تحمل غذاهای تند، خشکی دهان شده و در موارد شدیدتر، تریسموس، محدودیت در میزان بازکردن دهان، اختلال در حرکت زبان، بلع، تکلم و میکروستومیا همراه آن رخ می دهد. اتیولوژی عارضه فیبروز تحت مخاطی (SMF) به طور کامل شناخته نشده است و اکثراً مولتی فاکتوریال ذکر شده است. علاوه بر آن فیبروز تحت مخاطی وضعیتی نادر در کودکان محسوب می شود و عود پس از درمان آن شایع می باشد.
گزارش مورد: در مطالعه حاضر یک مورد نادر فیبروز تحت مخاطی یک طرفه با اتیولوژی نا شناخته در پسری 10 ساله به همراه روش ترکیبی خاص (دارویی/جراحی/پروتزی/ فیزیوتراپی) جهت درمان آن گزارش شده است.
نتیجه گیری: تشخیص صحیح و طرح درمان مناسب در بیماران فیبروز تحت مخاطی دهان بسیار ضروری است و عامل اصلی موفقیت و دستیابی به نتایج رضایت بخش درمانی می باشد.
