The peak incidence of this cyst is in the second and third decades of life. It tends to occur more in males and is often seen in the mandible, especially the posterior area. One of the important features of this lesion is its high expansion without clinical swelling. In cases where the lesion is small, it is usually asymptomatic; however, in larger cases, it may be accompanied by pain, swelling, or drainage.Case Report: A 42-year-old woman referred with a complaint of mandibular swelling. The patient described pain in some cases with low to moderate intensity without any specific pattern. An external oral examination showed face asymmetry with right mandibular swelling in the chin area. Intraoral examination revealed buccal expansion in the right mandible from dental mesial 32 to 46 mesial. All teeth involved in the lesion (32-46) responded positively to the cold test and pulp electrical test. In panoramic radiography, a unilocular radiolucent lesion with well-demarcated and corticated borders was observed in the mandible anterior from 32 to 46 mesial, and in the cone beam computed tomography radiographic view, the perforation of the buccal and lingual cortex was observed. The patient underwent incisional biopsy and marsupialization, and the sample was referred to the pathologist for histopathological examination. In the microscopic examination, histopathological characteristics of OKC were observed.Conclusion: Although the usual feature of OKC is a lesion without bone swelling, according to this case report, in the differential diagnosis of this lesion, lesions with expansion and perforation of the cortex should also be considered.}, keywords = {jaw cysts,Mandible,Odontogenic cysts,Odontogenic keratocyst}, title_fa = {ادنتوژنیک کراتوسیست در فک پایین همراه با تورم استخوانی و پرفوراسیون کورتکس: گزارش یک مورد}, abstract_fa = {مقدمه: ادنتوژنیک کراتوسیست (OKC)، یکی از شایع ترین کیست های ادنتوژنیک رشدی تکاملی می باشد و از بقایای سلولی دنتال لامینا منشا می گیرد. حداکثر بروز این کیست در دهه دوم و سوم زندگی می باشد. تمایل بیشتری به بروز در مردان دارد و اغلب در فک پایین به ویژه ناحیه خلفی دیده می شود. از ویژگی های مهم این ضایعه گسترش زیاد آن بدون ایجاد تورم بالینی می باشد. در مواردی که ضایعه کوچک است معمولا بدون علامت است، اما در موارد بزرگتر ممکن است همراه با درد، تورم یا درناژ باشد.گزارش مورد: بیمار خانم 42 ساله ای بود که با شکایت از تورم فک پایین مراجعه کرده بود. بیمار درد را در برخی مواقع با شدت کم تا متوسط بدون الگوی خاصی ذکر می نمود. در معاینه خارج دهانی، آسیمتری صورت همراه با تورم سمت راست مندیبل در ناحیه چانه مشاهده می شد. در معاینه داخل دهانی اکسپنشن باکالی در مندیبل سمت راست از مزیال دندان32تا مزیال  دندان 46 دیده شد. همه دندان های درگیر ضایعه (32-46) به تست سرما و تست الکتریکی پالپ پاسخ مثبت دادند. در رادیوگرافی پانورامیک یک ضایعه رادیولوسنت یونی لاکولار با حدود مشخص و کورتیکه در قدام مندیبل از مزیال دندان 32 تا مزیال دندان 46 و در نمای رادیوگرافی CBCT، پرفوراسیون کورتکس باکال و لینگوال مشاهده شد. بیمار تحت بیوپسی اینسیژنال و مارسوپیالیزیشن قرار گرفت و نمونه جهت بررسی هیستوپاتولوژی به پاتولوژیست ارجاع گردید. در بررسی میکروسکوپی، مشخصات هیستوپاتولوژیک ادنتوژنیک کراتوسیست (OKC) مشاهده شد. نتیجه گیری: اگرچه ویژگی معمول ادنتوژنیک کراتوسیست، ضایعه ای بدون ایجاد تورم استخوانی است، اما با توجه به مورد گزارش شده، در تشخیص افتراقی این ضایعه، ضایعات همراه با اکسپنشن و پرفوراسیون کورتکس را نیز باید مدنظر قرار داد .}, keywords_fa = {ادنتوژنیک کراتوسیست,کیست های ادنتوژنیک,کیست های فکی,مندیبل}, url = {https://jmds.mums.ac.ir/article_21935.html}, eprint = {https://jmds.mums.ac.ir/article_21935_fa8561c7fe89ed5c41465473b3dcae42.pdf} }
