In Pierre Robin sequence, the events occur in sequence, with one defect leadingto the next. PRS occurs with an incidence of 1 in 8500 to 25000. Affectednewborns have varying degrees of airway obstruction, often in association withgastroesophageal reflux and feeding difficulties. Intervention is mostly aimedat restoring a patent airway. Case Reports: Airway management in the infant withPierre Robin sequence can be very challenging because of difficulty with maskventilation and intubation. Our patient was a 4-year old boy with Robin sequence scheduledfor dental repair under general anesthesia. He had a history of palaterepair 3 years ago. After inhalation induction with sevofluran, nitrous-oxide inoxygen‚ under spontaneous ventilation, initial laryngeal view with theconventional Macintosh laryngoscope revealed Cormack-Lehane grade IIӀ due torestricted mouth opening and micrognathia. Nasal intubation for two timesunder the spontaneous respiration were unsuccessful. We fiberopticallyintubated the patient with uncuffed tracheal tube (ID 5 mm) while allowing himto breathe spontaneously. Extubation wasdone with the patient fully awake and with emergency airway equipmentimmediately available. Postoperatively, he was transferred to intensive careunit where he could be observed closely for delayed complications of airwayobstruction. Conclusion: Managing the airway of patients with craniofacialabnormalities can potentially be difficult. Therefore we recommend even simple treatment procedures to be carried out by experienced anesthesiologists, with a variety of different airwaydevices available such as flexible fiberoptic‚ Storz video laryngoscope and Airtraqoptical laryngoscope.}, keywords = {Pierre robin sequence,Complications,intubation tracheal,fiber optic technology}, title_fa = {اانجام بیهوشی عمومی برای خدمات دندانپزشکی یک بیمار مبتلا به Pierre Robin Sequence: گزارش مورد}, abstract_fa = {مقدمه:Pierre Robin Sequence که قبلاً به عنوان سندروم پیر روبین نامیده می‏شد، شامل سه ناهنجاری مادرزادی ماندیبول کوچک، عقب افتادگی زبان و شکاف کام می‏باشد. به طوری که نقص اولیه عامل ایجاد نقص بعدی است. نوزادان مبتلا به این بیماری با مشکلات انسداد راه هوایی، بازگشت محتویات معده به مری وتغذیه همراهند. مداخلات صورت گرفته اغلب جهت حفظ یک راه هوایی باز است. شرح مورد: یکی از مشکلات اصلی با توجه به کوچکی استخوان ماندیبول، احتمال انتوباسیون مشکل و نگهداری راه هوایی در این بیماران می‏باشد. بیمار ما پسر 4 ساله کاندید ترمیم دندانی تحت بیهوشی عمومی بود. وی سابقه یک نوبت جراحی ترمیم شکاف کام، 3 سال قبل از مراجعه داشت. با توجه به مشکل راه هوایی اینداکشن بیمار با حفظ تنفس خود به خودی با سوفلوران و اکسیژن و نیتروس اکساید صورت گرفت. ابتدا لارنگوسکوپی مستقیم با لارنگوسکوپ‏های مکینتاش مرسوم انجام شد که با توجه به محدودیت باز شدن دهان و میکروگناسی با دید مستقیم سختی انتوباسیون کورماک-لیهان گرید III تخمین زده شد و با دو نوبت تلاش با شکست در انتوباسیون همراه بود. سپس با کمک فایبر اپتیک قابل انعطاف در حالی که تنفس خود به خودی همچنان حفظ شد، موفق به انتوباسیون نازال با لوله شماره 5 بدون کاف شدیم. در پایان جراحی، کودک در وضعیت کاملا بیداروبا آماده بودن کلیه وسایل اورژانسی باز کردن راه هوایی اکستوبه شد و با توجه به ریسک مشکلات راه هوایی پس از عمل، یک شب در ICU بستری و روز بعد با حال عمومی مناسب مرخص گردید. نتیجه گیری: در بیماران با ناهنجاری‏های کرانیو فاشیال مادرزادی که احتمال مشکلات راه هوایی وجود دارد، بهتر است که حتی جراحی‏های سرپایی مثل ترمیم دندان‏ها تحت بیهوشی عمومی، در مراکز مجهز به وسایل مختلف انتوباسیون مشکل و با حضور متخصص بیهوشی متبحر انجام شود.}, keywords_fa = {پی یر روبین,عوارض,انتوباسیون,تکنولوژی فایبر اپتیک}, url = {https://jmds.mums.ac.ir/article_1879.html}, eprint = {https://jmds.mums.ac.ir/article_1879_5405733646e74cb08c8168a5928ac0b8.pdf} }
