بررسی مقایسه ای میزان دبری خارج شده در طی درمان مجدد ریشه بعد از کاربرد تکنیک‌های اینسترومنتیشن چرخشی و دستی

چکیده

مقدمه:
درمان مجدد ریشه، راهی جهت تصحیح عوامل ایجاد کننده شکست درمان قبلی میباشد. در طی سالیان اخیر، سیستمهای چرخشی با کاربرد مؤثر و امنتر فایلهای نیکل تیتانیوم، دریچه تازهای برای دندانپزشکان دنیا گشوده است. هدف از این مطالعه مقایسه مقدار مادةبیرون رانده شده از فورامن اپیکالی ریشه بعد از کاربرد دو تکنیک دستی و چرخشی (با یا بدون کلروفرم) جهت خارج کردن مواد پر کننده ریشه بود.
مواد و روش ها:
در این مطالعه مداخله ای آزمایشگاهی کانالهای ریشه  96 دندان تک ریشهای کشیده شدة انسان با استفاده از تکنیک
Passive step- back
در سه طول کارکرد مناسب، رد شده و کوتاه پرشده (زیرگروههای a , b, c) آماده شدند و با ترکیبی از روشهای تراکم طرفی سرد و عمودی گرم پر شدند. بعد از خا رج نمودن مواد پر شده کانال واینسترومنتیشنمجدد با وسایل مورد آزمایش (گروه A: فایلهای دستی K-type SS و کلروفرم ، گروه B : فایلهای چرخشیNi-Ti و کلروفرم و گروه C : فایلهای چرخشی نیکل تیتانیوم بدون کلروفرم) وزن دبری بیرون رانده شده محاسبه شدو میزان آن در گروههای مختلف با استفاده از آزمون آماری Unvariated ANOVA  آنالیز گردید.
 نتایج:
در همة گروهها میانگین وزن دبری بیرون رانده شده از37/. میلیگرمبیشتر نبود. در گروه B مقدار وزن دبری از گروه C و A بیشتر بود. اختلاف وزنها بین هر سه گروه و زیر گروه‌های با طول پر کردگی ریشه مناسب (a) و رد شده(b) و کوتاه پر شده (c) معنی دار بود(P<0.001).
نتیجه گیری:
وقتی از سیستم چرخشی با کلروفرم در هنگام درمان مجدد استفاده می‌شود و در موارد کانال‌های ریشه درمان شده با طول پر کردگی بیشتر از حد مطلوب(Over Filled) مواد پرکنندة بیشتری از انتهای ریشه بیرون رانده می‌شود.

چکیده [English]

Introduction:
Non-surgical retreatment is a way to correct the factors responsible for the failure of previous endodontic treatment.
In recent years, rotary instrumentation systems have created a new option for both clinicians and patients.
The aim of the present study was to quantify and compare the amount of debris extruded from apical foramen during endodontic retreatment using rotary and hand instrumentation techniques with and without use of chloroform.
Materials & Methods:
In this invitro experimental study, root canals of 96 extracted single-root human teeth were instrumented by passive step back technique in three different working lengths: optimum, over and under (three subgroups: a, b, c). they were then obturated by a combination of cold lateral and warm vertical compaction methods.
After removal of gutta-percha, re-instrumentation was performed in three groups as follows: group A; stainless steel K-type hand files plus chloroform, group B; rotary Ni-Ti files plus chloroform and group C; rotary Ni-Ti files without chloroform.
Finally, the weight of extruded debris was blindly determined. The results were analyzed by Univariate ANOVA.
Results:
In all groups, the mean weight for extruded debris was not more than 0.37 gr. The weight of extruded debris was ranked as: Group B>Group C>Group A. The differences between three groups and subgroups were statistically significant (P<0.001).
Conclusion:
The results of this study showed that the largest amount of extruded debris during retreatment was found after using rotary files with solvent and in canals with overfilled obturation.
