تاثیر دهانشویه های حاوری نانوذرات فلزی بر ثبات رنگ کامپوزیت رزین
مقدمه: نشان داده شده است که دهانشویههای حاوی نانوذرات فلزی میتوانند به صورتی موثر در کنترل پلاک دندانی عمل نمایند. هدف از این تحقیق بررسی ثبات رنگ یک نوع ماده کامپوزیت بعد از مجاورت با چند محلول کلوئیدال حاوی ذرات نانو بود. مواد و روشها: ابتدا نمونههای کامپوزیتی تهیه و به مدت 24 ساعت در آب مقطر نگهداری شدند. سپس رنگ تمام نمونهها بر اساس سیستم CIELAB تعیین گردید (مرحله اول؛ ابتدای کار). در مرحله بعد، نمونهها به مدت 24 ساعت در دهانشویههای حاوی نانوذرات اکسیدتیتانیوم، اکسید روی، نقره، اکسید مس، کلرهگزیدین (کنترل مثبت) یا آب مقطر (کنترل منفی) غوطهور شدند و مجدداً سنجش رنگ انجام شد (مرحله دوم؛ بـعد از قرار دادن در دهـانشویه). سپس سـطح نمونهها تـوسط مسواک بـرقی به مدت 1 دقیقه تمیز شد و نـمونهها برای سـومین بار رنگسنجی شدند (مرحله سوم؛ پس از مسواک زدن). میزان تغییر رنگ در میان گروههای مختلف با استفاده از آزمون واریانس یک عاملی (ANOVA) و پس آزمون توکی مقایسه گردید. (05/0=α) یافته ها: آزمون ANOVA تفاوت معنیداری را در تغییر رنگ بین مراحل مختلف درمان در میان گروههای مختلف نشان داد (05/0P<). تغییر رنگ بین مرحله اول و دوم در دهانشویه های نقره و کلرهگزیدین به صورت قابل توجهی کمتر از دهانشویههای اکسید مس و اکسید تیتانیوم و در تمام گروهها به صورت قابل توجهی بیشتر از آب مقطر بود (05/0P<). تغییر رنگ بین مراحل اول و سوم در تمام گروههای دهانشویه از لحاظ آماری در یک سطح و به صورت قابل توجهی بیشتر از گروه آب مقطر بود (05/0P<). نتیجه گیری: دهانشویههای حاوی نانو ذرات فلزی تغییر رنگی در حد کلرهگزیدین و بیشتر نشان دادند. مسواک زدن تأثیر اندکی در برداشتن رنگ از روی نمونههای کامپوزیت داشت.

بررسی توزیع تنش در طرح های مختلف اوردنچر متکی بر سه ایمپلنت به روش اجزای محدود
مقدمه: به علت مشکلات دنچرها، امروزه اوردنچرهای متکی بر ایمپلنت رواج یافتهاند. به رغم موفقیت بالای ایمپلنت، از دست رفتن ایمپلنت هنوز معضل اساسی است. افزایش نیروی وارد بر ایمپلنت فاکتور مهم تحلیل استخوان و از دست رفتن ایمپلنت میباشد. این مطالعه با آنالیز اجزای محدود اوردنچرهایی با سه ایمپلنت، به بررسی بهترین طراحی و ترکیب اتچمنت که باعث حداقل استرس در استخوان آلوئول شود، پرداخته است. مواد و روشها: در این مطالعه به روش اجزای محدود، مدل هندسی مندیبل به کمک دادههای سی تی اسکن تولید گردید و سه ایمپلنت ITI در میـدلاین و محل دندانهای پـرمولار اول قرار گرفت. کـلیه شرائط با اسـتفاده از نرم افزار اجزای محدود شبیه سازی شد. سه طرح درمان Bar-ball، Bar و Ball جهت ساپورت اوردنچر در نظر گرفته شد. حداکثر تنش فون مایزز در سطح کلیه اتچمنتها و استخوان ساپورت کننده با یکدیگر مقایسه شد و خمش مندیبل نیز در طرحهای مختلف اتچمنت با یکدیگر مقایسه گردید. یافته ها: بیشترین میزان استرس در استخوان، اطراف گردن ایمپلنت و Threadهای فوقانی بود. اتچمنتهای Ball و Bar-Ball به ترتیب بیشترین و کمترین میزان استرس رانشان دادند. بیشترین میزان استرس در اتچمنت Ball در سیستم Bar-Ball دیده شد و بیشترین میزان حرکت اوردنچر در اتچمنت Bar-Ball مشاهده شد. نتیجه گیری: نتایج نشان داد، طرح درمان Bar-Ball به بهای کاهش Stability اوردنچر، باعث کاهش انتقال استرس به استخوان اطراف ایمپلنت میشود و طرح درمان Ball به بهای افزایش استرس به استخوان اطراف ایمپلنت باعث افزایش Stability اوردنچر میشود.

بررسی انواع اباتمنتهای پیچی در محل اتصال با فیکسچر با روش المان محدود در موقعیت دندان لترال
مقدمه: هدف درمان ایمپلنتهای مدرن، علاوه بر موفقیتآمیز بودن کاشت ایمپلنت، فراهم آوردن عملکرد، زیبایی و پایداری، مشابه دندان طبیعی است. یکی از مشکلاتی که اغلب در ایمپلنتهای دندانی مشاهده شده و باعث عدم رضایت بیماران میگردد، شلشدن اباتمنت میباشد. لذا هدف این تحقیق، بررسی ویژگیهای مکانیکی، طراحی سطح تماس فیکسچر- اباتمنت، در سیستمهای ایمپلنتی با انواع اباتمنتهای پیچی (یک/دو تکه) بود. مواد و روشها: تحقیق حاضر، 5 نوع سیستم ایمپلنتی تجاری، با حدود 103 مدل سه بعدی مربوط به مجموعه فیکسچر- اباتمنت، را پس از مدلسازی دقیق و شبیهسازی تحت اثر بارهای استاتیکی، دینامیکی و سیکلی در موقعیت دندان لترال مورد تحلیل المان محدود قرار داد. یافتهها: به طورکلی اباتمنتهای پیچی یک تکه، مقاومت بالاتری نسبت به انواع همراه با پیچ نگهدارنده (دو تکه) در برابر شلشدگی نشان دادند. نتیجهگیری: توجه به طراحی قسمت اتصال و چگونگی طراحیهای داخلی فیکسچر و اباتمنت میتواند در میزان مقاومت به شل شدگی موثر باشد.

مقایسه اثرات آنتی باکتریال ماده ضد عفونی کننده ایرانی Disept با مواد ضد عفونی کننده Micro10 enzyme وHelvemed forte
مقدمه: ترکیبات آمونیوم چهارتایی گروهی از ترکیبات شیمیایی ضدعفونیکننده هستند که امروزه در ضدعفونی کردن سطوح و وسایل دندانپزشکی کاربرد زیادی دارند. در این مطالعه اثرات آنتی باکتریال ضدعفونیکننده Diseptبا Helvemed forte و Micro10 enzyme روی وسایل دندانپزشکی آلوده سازی شده با استافیلوکوک طلایی و کاندیداآلبیکانس مقایسه شد. مواد و روشها: در این مطالعه آزمایشگاهی، 88 فرز مستعمل به 4 گروه مساوی تقسیم و توسط اتوکلاو استریل شدند. سپس داخل سوسپانسیون استافیلوکوک طلایی و کاندیداآلبیکانس غوطهور شدند. فرزهای آلوده شده به میکروارگانیسمهای مورد نظر در گروه A درون Helvemed forte، گروه B در Micro10 enzyme و گروه C داخل Disept قرار داده شدند. پس از گذشت زمان مشخص، نمونهها خارج شده و در محیط کشت مایع قرار گرفتند، سپس محیطهای آلوده شده با استافیلوکوک در محیط جامد SBA و محیطهای آلوده شده با کاندیداآلبیکانس در محیط جامد سابورودکستروز آگار کشت داده شدند. گروهDبه عنوان گروه شاهد، بدون قرار گرفتن در داخل محلول ضدعفونیکننده کشت داده شد. در بررسی نتایج از نرم افزار SPSSو آزمون Chi-squareبا سطح معنی داری Pیافتهها: پس از مدت زمان سپری شده و بررسی نتایج کشت، در سه گروه مورد هیچگونه رشد میکروبی مشاهده نگردید ولی در گروه شاهد نتیجه کشت مثبت بود. نتیجه گیری: قدرت ضدعفونی کنندگی محلول ایرانی Disept در غلظت 5% مشابه محصولات Micro10 enzyme و Helvemed forte است.

ارزیابی مشکلات پریودنتال مولردوم ماگزیلاری پس از برداشتن پیوند استخوان از ناحیه توبروزیته
مقدمه: یکی از بهترین روشها برای بازسازی نواقص استخوانی در ناحیه دهان و دندان بیماران، استفاده از پیوند اتوژن است که هنوز به عنوان استاندارد طلایی جهت آگمنتاسیون نواحی دچار کمبود استخوان، در نظر گرفته میشود. مناطق داخل دهانی متعددی برای برداشت این گرفت استخوانی وجود دارد که توبروزیته ماگزیلا به دلیل دسترسی راحتتر و عوارض جراحی کمتر، محل مناسبی جهت برداشت مقادیر محدود استخوان میباشد. هدف از انجام این مطالعه، ارزیابی مشکلات پریودنتال مولر دوم ماگزیلاری پس از برداشتن پیوند استخوان از ناحیه توبروزیته بود. مواد و روشها: ۱۵ بیمار مراجعه کننده به بخش جراحی و ایمپلنت دانشکده دندانپزشکی مشهد، با محدوده سنی ۲۰ تا ۴۰ سال، پس از بررسی شرایط ورود و خروج، وارد مطالعه شدند. برای هر بیمار شاخصهای عمق پاکت، از دست دادن چسبندگی لثه، شاخص پلاک، شاخص لثه و خونریزی بعد از پروب کردن، در سه ناحیه شامل میددیستال، دیستوباکال و دیستولینگوال از هر دندان، در دو زمان قبل و ۶ ماه بعد از جراحی، اندازه گیری و ثبت شدند. در تحلیل داده ها از آزمون t زوجی، ویلکاکسون و مک نمار استفاده شد (05/0α=). یافتهها: میانگین عمق پاکت در دیستال مولر دوم ماگزیلا، در دوز مان قبل و ۶ ماه بعد از عمل، به میزان ۱۵/۴% کاهش یافته بود که این میزان کاهش، معنیدار بوده است (002/0P=). سه شاخص پلاک (002/0P=)، از دست دادن چسبندگی لثه 025/0=P و شاخص لثه ۰77/۰=P به صورت رتبه ای ارزیابی شدند که هر سه با گذشت زمان کاهش یافته بود. میزان خونریزی مشاهده شده در ۶ ماه پس از جراحی ۴/٢۱% کاهش داشت اما این میزان کاهش معنیدار نبود (۲۵۰/۰P=). نتیجه گیری: برداشتن گرفت استخوانی از ناحیه توبروزیته، منجر به ایجاد مشکلات پریودنتال برای مولر دوم ماگزیلاری مجاور این ناحیه، نخواهد شد و استخوان ساپورت کننده در دیستال آن، چنانچه در طی جراحی دچار صدمه شده باشد، دوباره بازسازی شده به طوری که عمق پاکت و عرض بیولوژیک نرمالی خواهیم داشت.

تاثیر چهار نوع سمان روی استحکام گیر روکش های استیل زنگ نزن در دندان های مولر دوم شیری
مقدمه: روکشهای استیل زنگ نزن مواد ترمیمی ارزشمندی برای ترمیم مولرهای شیری محسوب میشوند. سمانهای چسباننده ارتباط بین روکش و ساختار دندان آماده شده را فراهم میکنند. لذا هدف از این پژوهش بررسی و مقایسه تاثیر استحکام گیر روکشهای استیل زنگ نزن (SCC) دندان، هنگام استفاده از چهار نوع سمان مختلف برای چسباندن روکشهای استیل زنگ نزن بود. مواد و روشها: 48 دندان مولر دوم شیری به صورت تصادفی به چهار گروه تقسیم شده و تحت تراش استانداردSCC قرار گرفته و برای هر دندان روکش با سایز و گیر مناسب انتخاب شد. قبل از سمان کردن سیم 9/0 ارتودنسی به صورت U شکل به کراونها لحیم شد، سپس کراونهای هر گروه با یکی از چهار سمان پلیکربوکسیلات، زینکفسفات، گلاسآینومر و سمان رزینی چسبانده شد. سپس دندانها در بزاق مصنوعی قرار گرفته و برای 24 ساعت در انکوباتور ᵒc37 قرار گرفتند. پس از آن دندانها توسط دستگاه اینسترون با سرعت mm/min 2تحت کشش قرار گرفتند. نیرو به آرامی افزایش پیدا کرد تا زمانی که علایم جدا شدن کراون دندان مشاهده شد. سطح SSC با استفاده از روش گرافیکی محاسبه شد و میزان استحکام گیر به صورت نیرو بر واحد سطح (kg/cm2)محاسبه شد. داده ها با آزمون آماریOne-way ANOVA تجزیه و تحلیل گردیدند. (05/0α=). یافته ها: میانگین استحکام گیر در سمان پلیکربوکسیلات kg/cm23/3±2/17، زینکفسفات kg/cm28/3±8/19، گلاسآینومر kg/cm28/3±01/17 و سمان رزینی kg/cm27/3±27/19 بود. بر اساس آنالیز آماریOne-Way ANOVA بین چهار گروه اختلاف آماری معنیداری مشاهده نشد. (1/0P=) نتیجه گیری: استحکام گیر چهار سمان پلیکربوکسیلات، زینکفسفات، گلاسآینومر و سمان رزینی تفاوت چندانی با هم ندارد. بنابراین انتخاب سمان میتواند بر اساس نیازهای فردی بیمار و با توجه به قضاوت دندان پزشک صورت گیرد.

هم خوانی سن تقویمی و مورفولوژی مهره های گردنی در رادیوگرافی لترال سفالومتری در جمعیتی از ایران
مقدمه: در برنامهریزی درمانی برای بیماران ارتودنسی از مورفولوژی مهرههای گردنی برای بررسی بلوغ اسکلتی استفاده میشود. هدف این مطالعه بررسی رابطه سن تقویمی و مراحل بلوغ مهرههای گردنی بود. مواد و روشها: در این مطالعه توصیفی- تحلیلی، 320 لترال سفالوگرام افراد در محدوده سنی 15-7 سال (160 پسر و 160 دختر) در شهر بابل مورد بررسی قرار گرفتند. افراد به 8 گروه سنی تقسیم شدند. از روش تغییریافته Baccetti برای بلوغ مهرههای گردنی (CVM) استفاده شد. ارزیابی شامل بررسی چشمی ویژگیهای مورفولوژیکی سه مهره گردنی (مهرههای دوم، سوم و چهارم گردن) بود. آنالیز دادهها با استفاده از نرم افزار SPSS با ویرایش 20 انجام شد. (05/0P≤) معنیدار تلقی گردید. یافته ها: ضریب همبستگی اسپیرمن بین سن تقویمی و مراحل بلوغ مهرههای گردنی در کل نمونهها 841/0 (001/0P<)، در پسرها 839/0 (001/0P<)، در دخترها 873/0 (001/0P<) بود. نسبت درستنمایی مثبت برای تعیین مراحل بلوغ مهرههای گردنی در کلیه گروههای سنی، به جز گروه سنی 8-7 سال (در پسرها و دخترها) و گروه سنی 10-8 و 15-14 سال در دخترها، پایین بود. نتیجه گیری: نتایج این مطالعه نشان داد در جمعیت، مورد مطالعه روش CVM، در تعیین زمان شروع بلوغ قابل اعتماد است و علیرغم وجود ضریب همبستگی بالای بین سن تقویمی و مراحل بلوغ مهرههای گردنی، پایین بودن نسبت درستنمایی مثبت در بیشتر گروههای سنی، بیانگر این مسئله است که سن تقویمی نشانگر ضعیفی برای تعیین مراحل بلوغ است.

پیشگیری از تکثیر ویروس هرپس سیمپلکس تیپ 1 با عصاره آبی ریشه گیاه سرخارگل
مقدمه: کاربرد دارویی گیاهان برای درمان بیماریهای گوناگون در زندگی انسان بسیار دیده شده است. امروزه درمان آلودگیهای ویروسی با داروهای شیمیایی در دسترس با پیدایش مقاومت دارویی در ویروسها با چالشهایی روبرو شده است، لذا نیاز به داروهای ضدویروسی نوین احساس میگردد. در این پژوهش اثر بازدارندگی گیاه سرخارگل بر ویروس هرپس سیمپلکس تیپ 1 در زمانهای گوناگون اثردهی عصاره بررسی شد. مواد و روشها: نخست عصاره آبی ریشه این گیاه فراهم گردید و پس از سنجش آستانه توکسیسیته آن بر روی دودمان یاخته ای (Human epithelial type 2 Hep-2) با ارزیابی (Cytopathic effect) CPE، اثرضدویروسی عصاره گیاهی با روش Neutralization Test (NT) در زمانهای یک، دو و سه ساعت اثردهی عصاره سرخارگل پس از تلقیح ویروس هرپس سیمپلکس تیپ 1 بررسی گردید. یافته ها: عصاره گیاه سرخارگل در غلظتهای μg/ml 900-50هیچ گونه اثر کشندگی بر روی یاختهها نداشت. یافتهها نشانگر بیشترین ویژگی بازدارندگی این گیاه در یک ساعت پس از بردن ویروس بر روی یاختهها بود که در رقتهای μg/ml 900-500 بازدارنده کامل تکثیر ویروس بود، ولی این ویژگی ضدهرپسی در دو و سه ساعت کاهش یافت. نتیجهگیری: گیاه سرخارگل در غلظتهای غیرتوکسیک به کار رفته بر روی دودمانهای یاختهای، اثر ضدهرپسی خوبی را بر روی ویروس هرپس سیمپلکس تیپ 1 نشان داد. برای پیدا کردن سازوکار اثر این دارو نیاز به کارآزمایی بیشتر میباشد تا در ساخت داروهای ضدهرپسی به کار رود.

بررسی اثر خمیردندان حاوی عصاره آبی کلاله زعفران بر روی شاخصهای لثهای بیماران مبتلا به ژنژیویت مارژینال ژنرالیزه وابسته به پلاک دندانی
مقدمه:زعفران دارای انواع زیادی از ترکیبات فلاوونوئیدی، گلیکوزیدی و آنتوسیانینها می باشد که خواص ضدالتهابی، آنتیاکسیدان و غیره دارد. این تحقیق، به منظور بررسی اثرات زعفران بر روی شاخصهای لثهای بیماران مبتلا به ژنژیویت مارژینال ژنرالیزه وابسته به پلاک دندانی انجام شد. مواد و روشها: برای این مطالعه از خمیردندان حاوی عصاره آبی کلاله زعفران استفاده شد. 22 نفر بیمار که مبتلا به ژنژیویت مارژینال جنرالیزه بودند، انتخاب گردیدند. بیماران به صورت تصادفی به دو گروه 11 نفری تقسیم شدند. در یک گروه از خمیردندان زعفران و در گروه دیگر پلاسبو روزانه دو بار استفاده شد و برای هر گروه شاخص عمق پاکت (PD)، شاخص لثهای (GI)، شاخص پلاک (PI) و شاخص خونریزی هنگام پروب کردن (BOP) قبل از کاربرد خمیردندان و یک ماه بعد مورد ارزیابی قرار گرفت. آزمون های t زوجی مستقل، من-ویتنی و ویلکاکسون در تجزیه و تحلیل داده ها استفاده شد (05/0α=) یافته ها: در پایان مطالعه مقایسه شاخصهای اندازه گیری شده قبل و یک ماه پس از مداخله، در هر گروه، حاکی از کاهش معنیدار در تمامی شاخصها بود (01/0P<). گروه مداخله تفاوت معنیداری در کاهش شاخصهای لثه ای و خونریزی حین پرابینگ نسبت به گروه دارونما داشت (038/0P=). اما در شاخصهای PD و شاخص پلاک این تفاوت به لحاظ آماری معنیدار نبود (05/0P>). نتیجه گیری:خمیردندان حاوی عصاره آبی کلاله زعفران میتواند برخی از شاخصهای لثهای در بیماران مبتلا به ژنژیویت را بهبود بخشد.

بررسی گشتاور باز کردن پیچ اباتمنتهای یک قطعه ای و دو قطعه ای در شرایط خشک و مرطوب
مقدمه:استفاده از ماده لوبریکنت در حد فاصل رزوههای بین پیچ اباتمنت و ایمپلنت به منظور کاهش احتمال شل شدن پیچ اباتمنت پیشنهاد شده است. هدف از این مطالعه مقایسه گشتاور لازم برای بازکردن پیچ در اباتمنتهای یک قطعهای و دو قطعهای در شرایط خشک و آغشته به بزاق و پر شده از بزاقبود. مواد و روشها:10 ایمپلنت ITI در داخل بلوکهای آکریلی مانت شد. 10 اباتمنت یک قطعهای با نیروی گشتاور 10 و20 و 35 نیوتن سانتیمتر بر روی ایمپلنتهای مربوطه محکم شدند و نیروی لازم برای باز کردن آنها ثبت شد. این آزمایش در سه شرایط مختلف انجام شد: محیط خشک، آغشته کردن پیچ اباتمنت به بزاق مصنوعی و پر کردن حفره فیکسچر با بزاق مصنوعی. همین مراحل برای اباتمنتهای دو قطعهای نیز تکرار شد. نتایج توسط آزمون Repeated measure ANOVA و LSD آنالیز شدند. برای مقایسه گشتاور باز کردن اباتمنتها با گشتاورهای بستن مربوطه از آزمون One-Sample t-test استفاده شد (05/0 (α=. یافته ها:از محیط خشک به محیط آغشته به بزاق و محیط پر شده از بزاق میانگین گشتاور باز کردن اباتمنتهای یک قطعهای و دو قطعهای به تدریج و به طور معنیداری افزایش یافت (001/0P<). هنگام استفاده از هر سه گشتاور بستن Ncm 10، 20 و 35 میانگین گشتاور لازم برای باز کردن، برای اباتمنتهای یک قطعهای بیشتر از دو قطعهای بود (001/0P<). نتیجه گیری: در اباتمنتهای یک قطعهای و دو قطعهای، به ترتیب آغشته شدن پیچ اباتمنت با بزاق و پرشدن حفره داخلی فیکسچر با بزاق گشتاور لازم برای باز کردن پیچ اباتمنت را افزایش میدهد.
