Cessation in using warfarin 2-3 days before tooth extraction is one of the methods for preventing bleeding in these patients, but it may increase the risk of thromboembolism. The aim of this study was to use localized haemostatic drugs instead including gelatin sponge and tranexamic acid mouthwash. Materials & Methods: In this clinical trial study, 22 patients who were currently receiving warfarin with INR range of 2-4 were selected. For each patient 3 teeth were extracted in several times. After extraction of each tooth, Gelatin sponge, Tranexamic acid 4/8% mouth wash and sterile gauze (without any haemostatic drug) were used.Tendency of bleeding in 66 teeth, 2 and 24 hours after extraction was checked. No suture was used for any patient. Data were analyzed with descriptive statistics (frequency and average) and Chi-square and Fisher's exact tests. Results: Rate of bleeding in three groups were measured: Gelatin sponge: 9.1%, Tranexamic acid 4.8% mouthwash: zero and sterile gauze (without any haemostatic drug): 27.3% . Conclusions: In patients treated with warfarin, interruption in use of warfarin for tooth extraction is not needed. Local haemostasis with tranexamic acid 4.8% mouthwash without any suture is sufficient.}, keywords = {Warfarin,Tranexamic acid,Gelatin Sponge,INR}, title_fa = {بررسی تاثیر اسفنج ژلاتینی و شستن دهان با Tranexamic acid در جلوگیری از خونریزی پس از کشیدن دندان، در بیماران مصرف کننده وارفارین}, abstract_fa = {مقدمه:کنترل خونریزی در بیماران مصرف کننده داروهای ضد انعقادی مثل وارفارین همواره یکی از چالش‏های پیش روی دندانپزشکان بوده است. قطع مصرف وارفارین 3-2 روز پیش از کشیدن دندان از جمله راه‏های پیشگیری از خونریزی در این افراد است. اما این عمل همراه با افزایش خطر تشکیل لخته و ترمبوآمبولیسم می‏باشد. هدف از این مطالعه، استفاده از هموستازهای موضعی شامل اسفنج ژلاتینی و دهانشویه Tranexamic Acidبه جای قطع مصرف وارفارین بود. مقدار INR، از طریق محاسبهنسبت زمان پروترومبین ((PT بیمار و مقدار استاندارد کنترل به دست می‌آید. مواد و روش‌ها: در این مطالعه کارآزمایی بالینی، تعداد 22 بیمار مصرف‏کننده وارفارین با میزان INR 2 تا 4، انتخاب شدند. برای هر بیمار سه دندان در  زمان‏های  مختلف کشیده شد.  به ازای هر دندان کشیده شده  به ترتیب از :  اسفنج  ژلاتینی، 8/4% Tranexamic acid  (به صورت دهانشویه) و گاز استریل (بدون استفاده از هموستاز موضعی) برای کنترل خونریزی استفاده شد. در مجموع تمایل به خونریزی برای 66 دندان در زمان‏های 2 و 24 ساعت پس از کشیدن بررسی شد. برای هیچ کدام از روش‏های فوق از بخیه استفاده نشد. اطلاعات با استفاده از آمار توصیفی (فراوانی و میانگین) و آزمون کای دو و آزمون دقیق فیشر مورد تحلیل قرار گرفت. یافته‏ها:میزان خونریزی در گروه‏های اسفنج ژلاتینی، 8/4% Tranexamic acid و گاز استریل به ترتیب، 1/9%، صفر و 3/27% بود. نتیجه‏گیری:در بیماران مصرف‏کننده وارفارین نیازی به قطع وارفارین یا کاهش دوز آن نیست. استفاده از 8/4% Tranexamic acid به عنوان هموستاز موضعی بدون استفاده از بخیه موثر می‏باشد.}, keywords_fa = {وارفارین,Tranexamic acid,اسفنج ژلاتینی,INR}, url = {https://jmds.mums.ac.ir/article_975.html}, eprint = {https://jmds.mums.ac.ir/article_975_d603198791dc12262e9c6133aa34945e.pdf} }
